摘  要:目的探讨血浆8-异前列腺素F2а(8-iso-prostaglandin F2α,8-isoPGF2α)水平与早期糖尿病肾病的关系及临床意义。方法选择2型糖尿病患者63例,分为单纯糖尿病组(22例)和早期糖尿病肾病组(41例);以同期健康查体者31例作为正常对照组,用ELISA法检测3组血浆8-isoPGF2а水平,并进行统计学分析。结果单纯糖尿病组、早期糖尿病肾病组和对照组患者血浆8-isoPGF2а水平分别为(394.5±104.1)pg/ml、(562.7±211.8)pg/ml和(292.7±66.7)pg/ml,组间比较差异有统计学意义(F=13.907,P<0.001)。与对照组比较,单纯糖尿病组及早期糖尿病肾病组8-isoPGF2а水平显著升高,差异有统计学意义(P<0.05,P<0.001);与单纯糖尿病组比较,早期糖尿病肾病组8-isoPGF2а水平明显升高,差异有统计学意义(P=0.001)。血浆8-isoPGF2а水平与HbA1c呈显著正相关(r=0.349,P=0.012)。结论2型糖尿病肾病早期氧化应激水平明显增加,检测血浆8-isoPGF2а水平对于预测早期糖尿病肾病可能具有一定临床意义。Objective To study the relationship between plasma 8-isoPGF2α and early stage diabetic nephropathy and analyze it's clinic significance.Methods 63 patients with type 2 diabetes mellitus (DM) were divided into 2 groups according to the value of urinary albumin excretion rate (UAER):①simple DM group (SDM,UAER20μg/min),22 cases;②early diabetic nephropathy (EDN,UAER20~200μg/min),41 cases.The plasma level of 8-isoPGF2αwas measured in the type 2 DM patients of 2 groups and 31 normal controls(NC)by ELISA.Results ①The plasma level of 8-isoPGF2α in patients with SDM,EDN and NC groups were(394.5±104.1)pg/ml、(562.7±211.8)pg/ml and(292.7±66.7)pg/ml,respectively.Significant difference was found among the 3 groups (F=13.907,P〈0.001)②Compared to NC group,the level of 8-isoPGF2αwas markedly high in SDM and EDN groups (P〈0.05,P〈0.001).Further more,the level of 8-isoPGF2αwas higher in EDN than SDM (P=0.001)③ There was a significant positive correlation between 8-isoPGF2αand HbA1c(r=0.349,P=0.012).Conclusion The plasma level of 8-isoPGF2αwas significantly elevated in EDN.Measurment of plasma 8-isoPGF2αseems to be helpful for early diagnosis of diabetic nephropathy.
