脓毒症患者血清瘦素及相关细胞因子水平变化的初步研究

摘　　要：目的探讨脓毒症患者血清瘦素及相关细胞因子水平变化并初步探讨瘦素在脓毒症发病中的可能意义。方法测定34例脓毒症患者的血清瘦素、脂肪酸结合蛋白（FABP）、转铁蛋白（Ferr）及IL-1β、高灵敏的C反应蛋白（CRP）水平，分析瘦素与这些细胞因子的相关性。30例健康查体者作为对照组。结果脓毒症患者的血清瘦素、CRP、IL-1β、FABP和Ferr水平均显著高于对照组（均P〈0．01）。瘦素与FABP显著正相关，但与CRP、IL-1β和Ferr相关性不显著。结论脓毒症时血清瘦素、CRP、IL-1β、FABP和Ferr水平升高。瘦素与FABP显著正相关，这提示瘦素可能在脓毒症的发生过程中发挥一定的作用。Objective To investigate the serum level of leptin and other related cytokines in septic patients and the possible role leptin plays in the pathogenesis of sepsis. Methods Peripheral blood samples were collected from 34 septic patients and 30 healthy persons undergoing health examination, as control group, to detect the serum levels of leptin, fatty acid binding protein （FABP）, transferrin （Ferr）, interleukin-1β （IL-1β）, and C-reactive protein （CRP）. The correlation of leptein with the other cytokines was analyzed. Results The serum levels of leptin, CRP, IL-1β, FABP, and Ferr of the septic patients were all significantly higher than those of the normal controls （all P〈0.01）. The serum leptin level of the septic patients was significantly positively correlated with the level of FABP, but not correlated with the levels of the other cytokines. Conclusion The serum levels of leptin, CRP, IL-1β, FABP, and Ferr significantly increase in sepsis. Leptin may play a role in the pathogenesis and prognosis of sepsis.
