摘　　要：目的:完善处方评价制度,促进我院临床合理用药。方法:依据2007年5月1日实施的《处方管理办法》,随机抽取我院2007年5月～2008年12月门急诊处方2000张,并随机抽取小儿科、老年病科、呼吸科病区出院带药处方500张,运用Excel进行统计分析,对不合理处方和专科用药进行评价。结果:我院门急诊处方合格率88.7%,小儿科病区、老年科病区、呼吸科病区处方合格率分别为98.5%、97.0%、99.0%;不合格处方中处方前记占36.7%,处方量超过7(3)天未写原因占28.8%,配伍禁忌占12.8%,相互作用占不合格处方的9.7%,禁忌用药占不合格处方的6.6%。结论:我院抗菌药物、注射剂、基本药物等使用相对比较合理,配伍禁忌和药物相互作用应该得到重视,有待临床医生和临床药师的共同努力,提高我院合理用药水平。OBJECTIVE： To improve prescription evaluation system for promoting rational drug use in our hospital. METHODS： 2 000 outpatient prescriptions from May in 2007 to December in 2008,and 500 inpatient prescriptions from department of Pediatrics,Geriatrics and Respiratory in our hospital were sampled randomly for a statistical analysis using Excel in accordance with the＂Prescription Management Policy＂which was implemented since May 1,2007,and the irrational prescriptions and drug utilization in specialized departments were evaluated. RESULTS： The rates of qualified prescriptions in outpatient emergency department,pediatric department,senile department and respiratory department were 88. 7% ,98. 5% ,97. 0% and 99. 0% ,respectively. Of all the unqualified prescriptions,36. 7% were noted in preface,28. 8% had prescription amount of more than 7 （3） days yet without stating the reasons,12. 8% arose from incompatibility,9. 7% from drug interactions and 6. 6% from contraindication. CONCLUSION： The use of antibacterial drugs,injections,essential drugs was relatively reasonable in our hospital,yet incompatibility and drug interactions should be taken seriously,it calls for co - efforts of both clinicians and clinical pharmacists to improve the rational drug use level.
