非洛地平缓释片对老年原发性高血压患者大动脉顺应性的影响

摘　　要：目的观察非洛地平缓释片治疗轻、中度原发性高血压患者的降压疗效及其对大动脉顺应性的影响。方法选择轻、中度原发性高血压患者102例,根据年龄分为老年组(≥65岁,78例)和非老年组(<65岁,24例)。老年组患者又根据是否合并并发症分为并发症组(50例)和无并发症组(28例),其中并发症组合并糖尿病22例,合并脑梗死28例。比较各组患者应用非洛地平缓释片治疗12周前后血压和脉搏波速度(PWV)的变化。结果所有患者在应用非洛地平缓释片治疗12周后,其坐位血压及PWV均明显降低,差异有统计学意义。老年组PWV降低值明显高于非老年组,差异有统计学意义(P<0.05);并发症组与无并发症组的PWV降低值差异无统计学意义,合并糖尿病与合并脑梗死的患者PWV降低值差异无统计学意义。结论轻、中度原发性高血压患者在应用非洛地平缓释片治疗12周后,血压降低的同时,其大动脉顺应性亦改善,老年患者获益更多,且在老年患者中有并发症及无并发症的患者获益相同,合并糖尿病与合并脑梗死的患者获益相同。Objective To investigate the changes in pulse wave velocity（PWV） and blood pressure in sitting position after 12-week treatment with felodipine SR in elderly patients with mild to moderate primary hypertension and analyze the effect of felodipine SR on the large artery compliance. Methods 102 patients with mild to moderate primary hypertension were divided into two groups,old（≥65y, n=78） and non-old（〈65y, n=24）. The old group was further divided into with complications （n=50） and without complications （n=28）. There were 22 patients with diabetes and 28 with stroke in the old group. The changes in blood pressure and PWV after 12-week antihypertensive treatment with felodipine SR were compared between different groups. Results After 12-week treatment with felodipine SR,reduction in BP and PWV was observed in all patients, the difference was statistically significant. The value of reduction in PWV was higher in old group than in non-old group. However within the old group, there was no statistically significant difference in the reduction in PWV between with complication and without complication groups, the same result was observed in diabetic and stroke old patients. Conclusion After 12-week treatment with felodipine SR,reductin in blood pressure and improvement in large artery compliance were observed in patients with mild to moderate primary hypertension, greater benefit was obtained in elderly patients.
