曲美他嗪治疗扩张型心肌病心功能不全30例临床观察

摘　　要：目的观察曲美他嗪治疗扩张型心肌病心功能不全的临床疗效及安全性。方法选择58例扩张型心肌病心功能不全患者随机分为治疗组30例和对照组28例。对照组给予正规抗心力衰竭治疗;治疗组在对照组常规治疗基础上加服曲美他嗪20 mg,每日3次。2组疗程均为6个月。观察2组治疗前后临床疗效、左室舒张末内径(LVEDD)、左室射血分数(LVEF)、血清B型尿钠肽(BNP)、6 min步行试验及药物不良反应等。结果治疗6个月后,治疗组的总有效率93.3%显著高于对照组75.0%(P<0.05);在LVEDD、LVEF、6 min步行距离结果及BNP改善的程度等方面,治疗组均优于对照组(P<0.05);治疗组发生药物不良反应少而轻微。结论扩张型心肌病心功能不全患者长期服用曲美他嗪可使心功能改善,安全有效。Objective To observe the clinical efficacy and safety of trimetazidine for treatment of dilated cardiomyopathy with heart failure. Methods Fifty-eight cases of dilated cardiomyopathy with heart failure were randomly divided into treatment group （30 cases） and control group （28 cases）. The control group was applied conventional anti - heart failure therapy. The treatment group was given trimetazidine （ 20 rag, three times a day） based on the conventional therapy. After 6 months therapy courses, the left ventrieular end-diastolic diameter （LVEDD）, left ventrieular ejection fraction （LVEF） , serum B-type natriuretie peptide （BNP） , 6 min walk test, and adverse drug reactions of the two groups were evaluated pre and post treatment. Results After 6 months treatment, the total effective rate of the treatment group （93.3%） was significantly higher than the control group （75.0% , P 〈0.05 ）. As for the cardiac function indexes, included LVEDD, LVEF, BNP and 6 min walking distance, the treatment group were better than those in the control group （ P 〈 0.05 ） ; Adverse drug reactions was slightly in the treatment group. Conclusion Long-term use of trimetazidine could improve cardiac function of dilated cardiomyopathy with heart failure, which is safe and effective.
