摘 要:目的分析妊娠期肝内胆汁淤积症(ICP)母儿血清甘胆酸(CG)水平和胎盘中凋亡蛋白Caspase-3表达的关系,探讨ICP导致胎盘细胞凋亡的可能机制。方法以30例ICP患者及21例正常产妇为研究对象,用放射免疫法测定血清CG的水平,用免疫组化方法测定胎盘上凋亡调节基因Caspase-3的表达水平。结果①ICP组母血和脐血CG水平分别为:(16946.00±5601.30)ng/ml、(5925.43±847.90)ng/ml,对照组分别为(1467.91±290.90)ng/ml、(2573.75±363.62)ng/ml,二者差异均有统计学意义,(P<0.01)。②ICP组母血CG水平明显高于脐血,且与脐血CG水平呈正相关(r=0.87,P<0.05)。③ICP组胎盘组织中细胞滋养细胞、合体滋养细胞、蜕膜细胞及间质细胞中Caspase-3表达分别为(32.86±6.02)%、(21.57±7.52)%、(27.29±4.89)%与(18.14±3.58)%,明显高于对照组的(14.58±3.41)%、(9.58±2.26)%、(15.00±5.39)%与(9.17±2.20)%,P<0.05;④ICP组胎盘组织上述细胞中Caspase-3的表达与母血CG水平均呈正相关性,r分别为:0.81、0.59、0.57与0.43,P<0.05。结论①ICP组母儿血清CG水平均升高,母血明显高于脐血,且与脐血CG水平呈正相关。②ICP组胎盘组织细胞中Caspase-3的表达与母血CG水平呈正相关。③ICP组胎盘功能减退与细胞凋亡密切相关,Caspase-3基因可能是ICP胎盘细胞凋亡的重要调控基因之一。Objective To investigate the relationship between the levels of serum cholyglycine(CG) of mother and fetus and the expressions of CasPase-3 in placenta with intrahepatic cholestasis of pregnancy(ICP).Methods Samples were obtained from 30 cases with ICP(ICP grouP) and 21 normal pregnant women(control group).The serum CG were measured by radio-immunity method.The expressions of Caspase-3 in placenta tissues were detected by immunohisto-chemistry method.Results ① The levels of serum CG of mother and fetus of ICP group were significantly higher than those of control group,P0.01.② The level of serum CG of mother was higher than that of fetus,and there was a positive correlation in CG level between maternal serum and fetal umbilical cord serum in ICP group(r =0.87,P0.05);While,the level of CG in fetal umbilical cord serum was higher than that in maternal serum in control group.③ The expressions of Caspase-3 in placental cytotrophoblast,syncytitrophoblast,decidual cells and mediate cells in ICP group were were significantly higher than those in the control group P0.05;④ There was a significantly positive correlation between the level of serum CG of mother and the expression of Caspase-3 in Placental syncytitrophoblast、cytotrophoblast、decidual cells and mediate cells in ICP group(r=0.81、0.59、0.57、0.43,P0.05).Conclusion The levels of serum cholyglycine(CG) of mother and fetus is related to the expressions of CasPase-3 in placenta with intrahepatic cholestasis of pregnancy,the abnormal expressions of Caspase-3 in placental tissue with ICP could be one of the main reasons for Placental cells apoptosis.
