摘　　要：目的探讨CT引导下靶位注射胶原酶联合O3椎间盘溶解术中突出物CT值与疗效的关系。方法回顾性研究自2008年6月至2009年6月收治的病例100例。CT引导下穿刺,以突出物为"靶",选择穿刺层面及穿刺路径,穿刺成功后于靶内外注射胶原酶。结果所有患者全部一次穿刺成功。术后6个月随访,CT值≤90组:优32例,良23例,可3例,差2例,优良率91.7%,有效率96.7%;CT值>90组:优5例,良7例,可9例,差19例,优良率30.0%,有效率52.5%。结论CT引导下靶位注射胶原酶联合O3椎间盘溶解术中,突出物的CT值与疗效呈明显负相关,可作为选择治疗方法及预测术后疗效的客观指标。Objective To investigate the correlation between CT value and surgery effect of target-injection of collagenase and O3 chemonucleolysis by CT guidance for the treatment of lateral lumbar disc herniation.Methods 100 patients with LDH from June 2008 to June 2009 were analyzed retrospectively.The operation was carried out under CT guidance,and the target was the prominence.Collagenase was injected by the percutaneous hernia-puncture with the multipurpose spondylous needles.Results The successful rate of the first puncture was 100%.Among patients with CT value ≤90： The excellent and good rate was 91.7%,and the effective rate was 96.7%;Among patients withCT value 90 ： The excellent and good rate was 30.0%,and the effective rate was 52.5%.Conclusion During the target-injection of collagenase and O3 chemonucleolysis by CT guidance for the treatment of lateral lumbar disc herniation,the prominence CT value can provide some objective indicators for the surgery curative effect.
