心脏再同步化治疗8例充血性心力衰竭的疗效观察

摘　　要：目的评价心脏再同步化治疗(CRT)充血性心力衰竭患者的临床疗效。方法8例符合CRT治疗的充血性心力衰竭患者成功的行三腔起搏治疗,术前采用超声心动图检查进行收缩不同步的评价,术前和术后6个月进行美国纽约心脏病学会(NYHA)心脏功能分级评价、心电图检查。临床有效者定义为:术后6个月NYHA心功能分级改善1级以上的患者、超声心动图心室收缩末容积(LVEDD)缩小>15%或左室射血分数(LVEF)绝对值增加>5%的患者。结果8例患者6个月时胸闷气急临床症状明显改善,6例心脏功能达到NYHAⅡ级,2例NYHAⅢ级;LVEF绝对值较术前提高>5%。结论CRT通过改善心脏收缩的同步性改善心力衰竭患者的左室收缩功能,提高患者的生活质量和运动能力。Objective To observe and appraise the clinical curative effect of cardiac resyncronization therapy（CRT） to chronic congestive heart failure patients.Methods Cardiac resyncronization therapies were successfully performed in 8 patients with chronic congestive heart failure,adopting the ultrasonic echocardiography examination and EKG appraise before the operation,and carrying out the appraise of heart function hierarchic valuation and echocardiography examination of USA New York Heart Disease Association（NYHA） before implantation and postoperative six months to each patient.Results The clinical symptoms such as choking sensation in chest and breath with difficulty;were improved significantly in the 8 example patients;the heart function reached NYHAⅡ in the 6 example patients and another two patients with NYHA Ⅲ;the absolute value of LVEF was increased 5% compare with pre-operational value.Conclusion CRT could improve the heart contracting function in cardiac failure patients and enhance their life quality and exercise capacity.
