朝鲜族和汉族人群不同高血压亚型临床特征的比较

摘　　要：[目的]研究延边农村地区朝鲜族和汉族人群各高血压亚型的临床特征及高血压发生的影响因素.[方法]给3 011例30~70岁的朝鲜族及汉族人群进行血压、身高、体重、腰围（WC）、臀围、血糖及血脂等测定.根据收缩压和舒张压值分为正常血压（NBP）组、单纯收缩期高血压（ISH）组、单纯舒张期高血压（IDH）组及收缩期高血压合并舒张期高血压（SDH）组,进行比较分析.[结果]朝鲜族高血压人群中ISH,IDH,SDH亚型的构成比分别为12.33%,30.83%,56.84%,汉族中分别为14.70%,33.50%,51.80%,均以SDH最多见,而朝鲜族及汉族高血压亚型分布间略有差别.与NBP组比较,朝鲜族人群ISH组甘油三酯（TG）值显著升高,汉族人群ISH组腰围臀围比值（WHR）及腰围身高比值（WHtR）显著升高;朝鲜族及汉族人群IDH组体重指数（BMI）,WC,WHR,WHtR,血清总胆固醇（TC）,TG值显著升高;朝鲜族及汉族人群SDH组BMI,WC,WHR,WHtR,TG,FBG值显著升高.朝鲜族及汉族ISH组年龄均显著高于SDH及IDH组,IDH组年龄最低,朝鲜族IDH和SDH组BMI值均明显高于ISH组,SDH组WC,WHtR值明显高于ISH组.[结论]延边农村地区朝鲜族及汉族人群高血压亚型的分布间存在差异,均以SDH最多见,而总体脂含量、体脂分布、血糖及血脂是高血压发生的影响因素.OBJECTIVE To investigate the clinical characteristics of hypertension subtype and the influencing factors of hypertension in the Korean-Chinese and Han nationality of countryside of the Yanbian.METHODSA total of 3011 subjects aged 30～70 years were selected,and their blood pressure,weight,height,waist and hip circumference,blood glucose and lipid etc.were measured.All subjects were divided into the groups of normal blood pressure（NBP）,isolated systolic hypertension（ISH）,isolated diastolic hypertension（IDH） and systolic combined diastolic hypertension（SDH）.RESULTSThe constituent ratios of ISH,IDH and SDH were 12.33%,30.83% and 56.84% among Korean-Chinese with hypertension,and 14.70%,33.50% and 51.80% among Han nationality,respectively,of which the SDH was more frequent.As compared with the NBP subgroup,triglyceride was significantly increased in ISH among Korean-Chinese nationality,and WHR and WHtR were significantly increased in ISH among Han nationality;BMI,WC,WHR,WHtR,TC and TG were significantly increased in IDH among Korean-Chinese and Han nationality;BMI,WC,WHR,WHtR,TG and FBG were significantly increased in SDH among Korean-Chinese and Han nationality.In comparisons among three hypertension subtypes,the subjects with ISH were characterized by aging and the average age of IDH was the lowest,while IDH and SDH by increment of BMI,and SDH by increment of WC and WHtR.CONCLUSIONThe distribution of hypertension subtype among Korean-Chinese and Han nationality of countryside of the Yanbian was different,and the SDH is major subtype,and the total body fat,distribution of fat,hyperglycemia,blood glucose and lipid are the influencing factors of hypertension.
