摘  要:目的:本研究组以往的工作已证实血管钠肽(Vasonatrin peptide,VNP)对碾压撕脱伤皮瓣有明显的保护作用,本研究拟探讨该作用是否通过抑制皮瓣损伤后白细胞的L-选择素表达介导。方法:24只大鼠随机分为3组(n=8),分别为SHAM手术组、碾压撕脱组及VNP+碾压撕脱组,于术前及术后12h取材,HE染色观察组织学改变和白细胞浸润情况,并通过免疫组织化学染色对表达于白细胞表面的L-选择素进行标记以明确VNP对碾压撕脱皮瓣的具体作用。结果:碾压撕脱组皮瓣组织损伤情况较重,可见大量中性粒细胞浸润,其表面的L-选择素表达也随之升高,而VNP治疗组皮瓣中的中性粒细胞数目明显减少,其所表达的L-选择素也较前者显著降低(P<0.05)。结论:VNP可抑制白细胞表面L-选择素的表达,显著抑制碾压撕脱伤后皮瓣局部白细胞的活化,减轻其粘附和浸润,进而减轻炎症反应,抑制血栓形成,从而为临床上提高碾压撕脱皮瓣成活率并改善预后提供一种新的治疗思路。Objective Previous studies of our group showed that the Vasonatrin peptide (VNP)had protective effect on the cursh-avulsion injured skin flap. This study was designed to investigate whether the protection effect of VNP conducted via inhibition of expression of L-selectin on leucocytes. Methods Twenty four rats were randomly divided into three groups: the SHAM operation group, the crush injury group and the VNP treated group. Tissue samples from skin flap were collected before surgery and 12h after surgery, Hematoxylin-eosin stain was used to observe the histological change and inflammatory cell infiltration. Immunohistochemistry was applied to detect the expression of L-Selectin on leucocytes in the skin flap of each group, by which we can affirm the effects of VNP. Results Compared with the VNP treated group, tissue damage was more serious and a mass of inflammatory cell infiltration was observed, and the expression of L-Selectin was significantly higher (P﹤0.05)in the crush injury group. Conclusion VNP,which could mitigate inflammatory reaction and inhibit thrombosis by decreasing the inflammatory cell infiltration and down regulating the expression of L-Selectin in the crush injury, has the potential to be a new therapeutic drug for clinical application.
