Survivin、Caspase-3在宫颈非典型鳞状细胞中表达及意义

摘　　要：目的:采用液基薄层细胞学(TCT)诊断为宫颈非典型鳞状细胞(atypical squamous cell of undermined significance,ASCUS)的宫颈脱落细胞标本进行caspase-3、survivin免疫组织化学检测,探讨Caspase-3、survivin在ASCUS中的表达及其意义。方法:采用免疫组织化学SP法,检测139例ASCUS宫颈脱落细胞及10例正常宫颈细胞、20例宫颈炎宫颈细胞、4例鳞癌宫颈细胞中caspase-3、survivin蛋白的表达。结果:caspase-3、survivin在正常组与高度鳞状上皮内病变(high-grade squamous intraepithelial lesion,HSIL)、鳞癌(SCC)之间差异均有显著性;炎症与低度鳞状上皮内病变(low-grade squamous intraepithelial lesion,LSIL)、HSIL、鳞癌之间差异均有显著性(P<0.05)。Caspase-3、survivin在LSIL、HSIL、鳞癌之间阳性率差异均无显著性(P>0.05)。caspase-3、survivin在宫颈非典型鳞状细胞中表达呈负相关(P<0.01)。结论:caspase-3、survivin,在宫颈非典型鳞状细胞中的表达异常,为临床宫颈癌前病变的筛查提供更早期、更客观的检查手段,对于提高宫颈上皮内瘤样病变(CIN)的阳性检出率有重要意义。Objective： To investigate the expression and clinical significance of survivin and caspase - 3 in cervical atypical squamous cells. Methods： The expression of survivin and caspase - 3 protein were detected by immunohistochemical S - P method in 139 cases of ASCUS and 10 cases nor- mal squamous ceils and 20 cases of inflammation and 4 cases of cervical cancer. Results：The positive rates of caspase - 3 and survivin had significant difference between the normal group and high - grade squamous intraepithelial lesion （HSIL）（ P 〈 0.05 ） ; There were the significant differences among in- flammation and LSIL and HSIL. The positive rate of caspase - 3 and survivin were no significant difference（P 〉0.05）in LSIL and HSIL. The expression of caspase -3 was negatively correlated with survivin in cervical atypical squamous eells（P 〈 0.05 ）. Conclusion：The expression of caspase -3 and survivin in atypical squamous ceils may play an important role in the process of carcinogenesis and may be indentified as a defining diagnostic maker in cervical cancer.
