电针抗抑郁疗效作用特点分析

摘　　要：目的观察电针与氟西汀治疗抑郁症6周后的总体疗效,探讨两者对抑郁症患者不同症状因子的改善程度,分析两者疗效作用特点。方法60例抑郁症患者随机分为电针组和对照组,电针组主穴取百会、印堂,平刺,得气后接韩氏电针仪,频率为2/15 Hz,强度以患者耐受为度,电针时间30 min,每日1次,连续6周。对照组:氟西汀按常规用量(20 mg/d)连续服药6周。在治疗前和治疗后2周、4周、6周时用汉密尔顿量表(HAMD-17)对2组患者进行评定。结果治疗6周后,电针与氟西汀对抑郁症患者总体疗效无显著性差异(P>0.05)。电针组在治疗2周、4周后对焦虑/躯体化、睡眠障碍因子的改善程度与对照组相比有显著性差异(P<0.05)。对照组在治疗2周后、4周后对阻滞因子的改善程度与电针组相比有显著性差异(P<0.05),但在治疗6周后2组对各因子的改善没有显著性差异(P>0.05)。在认知障碍和体重因子方面,2组对其改善程度在治疗后各个时间段相比都没有显著性差异(P>0.05)。结论电针和氟西汀药物治疗抑郁症疗效相当,但两者疗效作用特点不同。治疗2～4周时,电针对睡眠障碍、焦虑/躯体化改善相对明显,而氟西汀对阻滞因子改善相对明显,直至6周两者对不同因子改善程度相当。Objective To observe the total curative effects of electroacupuncture and fluoxetine in the treatment of depression for 6 weeks,investigate the relief of symptoms in the patients and analyse the effective characteristics of two therapies.Method The patients with depression（n=60） were randomly divided into the electroacupuncture group and control group.The electroacupuncture group was treated with normal needling at the first in the acupoints of Baihui（GV 20） and Yintang（EX-HN 3）,and then stimulated with HANS electroacupuncture instrument after obtaining qi（2/15 Hz for 30 minutes and once a day） for 6 weeks.The control group was treated with fluoxetine（20 mg/d） for 6 weeks.Hamilton Depression Scale（HAMD） was used to review the patients of two groups before the treatment and 2,4 and 6 weeks after the treatment.Result After 6 weeks there was no significant difference in the total curative effect between two groups（P〉0.05）.In the electroacupuncture group the relief of factors of anxiety/ somatization and dyssomnia was different significantly to those in the control group（P〈0.05） 2 and 4 weeks after the treatment.In the control group the relief of retardation factor is different significantly to that in the electroacupuncture group（P〈0.05） 2 and 4 weeks after the treatment,but there was no significant difference in the above factors between two groups 6 weeks after the treatment（P〉0.05）.There was no significant difference in the factors of cognitive disorder and body weight between two groups（P〉0.05） at all time points.Conclusion Electroacupuncture and fluoxetine have the same curative effect on depression,but have different characteristics,that is,2 and 4 weeks after the treatment the former has more significant effects on dyssonmia and anxiety/somatization,and the latter has more significant effect on retardation factor.Two therapies have the same effects on all factors until 6 weeks after the treatment.
