摘  要:目的:观察持续声门下吸引预防机械通气(MV)发生呼吸机相关性肺炎的临床价值。方法:将ICU 72例气管插管行机械通气的患者随机分为观察组36例、对照组36例,对照组采用常规气道护理,观察组在此基础上行持续声门下吸引,收集口咽部、声门下、下呼吸道分泌物进行细菌学检查。结果:机械通气5 d内观察组呼吸机相关肺炎发生率明显低于对照组;观察组声门下分泌物培养革兰阳性致病菌数明显少于对照组。结论:持续声门下吸引可减少和延迟呼吸机相关性肺炎的发生。Objective:To study the clinical effect of continuous aspiration of subglottic secretion(CASS) on ventilator-associated pneumoni(VAP) in machanically ventilated(MV) patients.Methods:Divide 72 patients undergoing mechanical ventilation via tracheal intubation in ICU into observation group(n=36) and control group(n=36).Give control group routine inake-ports nursing care.On this basis,give observation group continuous aspiration of subglottic secretion.Collect the scraping-pharynx specimen,the subglottic secretion,and the lower airway secretion and make bacteriological test.Results:The comparison of the two groups:in 5 days of mechanical ventilation:the ventilator-associated pneumonia in observation group is obviously lower than in control group;ventilator-associated pneumonia appears later;Gram-positive bacteria in subglottic exudates is obviously less than in control group.Conclusions:Continuous aspiration of secretion can control and delay the ventilator-associated pneumonia.
