B超引导下应用气管异物钳取宫内节育器的效果分析

摘　　要：目的对B超引导下气管异物钳应用于困难取器做系统分析比较,评价其临床效果及适宜人群。方法收集困难取器对象90例,随机分为3组,B超引导下气管异物钳组、长弯血管钳组、多功能取环钳组,填写调查问卷,观察扩宫率、手术时间、术时疼痛反应、出血量、手术费用、器械成本和满意度评分。结果扩宫率在气管异物钳组最低,为3.33%,与其余两组相比,差异有显著性(P<0.01);手术时间气管异物钳组最短,为4.37±3.82min,3组间两两比较,差异有显著性(P<0.01);出血量长弯钳组最多,为11.12±4.49ml,与其余两组比较,差异有显著性(P<0.05);气管异物钳组与多功能取环钳组比较,差异无显著性(P>0.05);痛觉评分在气管异物钳组最低,为1.57±0.28分,与其余两组比较,差异有显著性(P<0.05);病人费用在气管异物钳组最高,为316.6±15.4元,与其余两组比较,差异有显著性(P<0.05);购置器械费用在气管异物钳组最高,为1200元,与其余两组比较,差异有显著性(P<0.05);满意度评分在气管异物钳组最高,为96.96%,与长弯钳组比较,差异有显著性(P<0.05),与多功能取环钳组比较,差异无显著性(P>0.05)。结论B超引导下气管异物钳应用于困难取器在使用新型宫内节育器的妇女和未生育妇女中值得尝试和推广,但须考虑患者的经济能力。OBJECTIVE To analysing and comparing the method of applying tracheal foring by ultrasound guiding in difficult IUD extraction patients,evaluating the clinic effect the accommadate woman. METHOD 90 cases of women with difficult IUD extraction experience were divided ininto three groups,tracheal foring group,blood vessel forcep group and multifuction cramp group. Qustionnare,rate of cervix expanding,operation time,pain reactin,volume of blood,operation cost,instrument cost,satisfaction degree were recorded. RESULTS The rate of cervix expanding in tracheal foring group was the lowest 3.33% which was significant difference compared to the other two groups（P0.01） . Operation time in tracheal foring group was the shortest 4.37±3.82 min. There was significant difference between three groups（P0.05） . Volume of bleeding in blood vessel forcep group was the most 11.12±4.49 ml,which was significant difference compared to the other two groups（P0.05） . There was no significant difference between tracheal foring group and multifuction cramp group（P0.05） . Pain reactin score in tracheal foring group was the lowest 1.57±0.28,with significant difference compared to the other two groups（P0.05） . Operation cost in tracheal foring group was the highest 316.6±15.4,with significant difference compared to the other two groups（P0.05） . Instrument cost in tracheal foring group was the highest 1 200,with significant difference compared to the other two groups（P0.05） . Satisfation degree in tracheal foring group was the highest 96.96%,with significant difference compared to blood vessel forcep group（P0.05） ,but there was no significant difference compared to multifuction forcep group（P0.05） . CONCLUSION Applying tracheal foring by Ultrasound Guiding in difficult IUD extraction patients was suitable for using new type IUD women and nonparagrative woman. Economic conditon of patients should also be considered.
