摘 要:目的筛选细粒棘球蚴病患者血清蛋白质组学标记物,探索其用于细粒棘球蚴病诊断的价值。方法细粒棘球蚴病患者35例,非棘球蚴病其他肝病对照组17例,健康对照组20例,用弱阳离子交换芯片(CM10芯片)结合表面增强激光解析离子化飞行时间质谱(SELDI-TOF-MS)技术检测血清蛋白质谱,用ZUCI-蛋白芯片数据分析系统软件包进行分析,通过支持向量机运算筛选病例组和对照组血清差异表达蛋白峰,建立细粒棘球蚴病蛋白质指纹图诊断模型并用留一法验证其诊断价值。结果细粒棘球蚴病患者和健康对照组之间共筛选出8个差异表达蛋白峰,质荷比分别为1044、1046、1055、1075、6452、6649、8714的几个蛋白峰在病例组中下调,质荷比为5651的蛋白峰在病例组中上调(P<0.05);细粒棘球蚴病组和其他肝病对照组之间筛选出1个差异蛋白峰(质荷比为5951,P<0.05);其他肝病组和健康组合并后的对照组和细粒棘球蚴病组之间共筛选出8个差异蛋白峰,留一法验证表明以此差异蛋白峰建立的诊断模型灵敏度为62.86%,特异度为67.57%。结论SELDI-TOF-MS蛋白质芯片技术能筛选出细粒棘球蚴病患者血清中的差异表达蛋白质,有望能成为细粒棘球蚴病的诊断和研究方法。Objective To filter serum proteomic signs of patients with cyst echinococcosis(CE),and explore their value for diagnosis of cyst echinococcosis.Methods Serum samples from 35 patients with cyst echinococcosis confirmed by surgery and pathology,17 patients with other liver disease and 20 normal humans were analyzed using SELDI-TOF-MS and CM10 Protein Chip.ZUCI-Protein Chip Data Analyze System was used to analyze the different protein peak spectrum expressed in serum of patient group and control group,established and evaluated the value of the model for diagnosis of cyst echinococcosis.Res-ults There were eight different protein peak spectrums between CE group and normal group,of which the 1044,1046,1055,1075,6452,6649,8714 m/z peak spectrum were decreased and the 5651 m/z peak spectrum was increased in patients group(P〈0.05).There was one different protein peak spectrum between CE group and other liver disease group(5951 m/z,P〈0.05).There were eight different protein peak spectrums between CE group and control group combined by normal group and other liver disease group.Leave one method demonstrated that the sensitivity and specificity of the model based those different protein peak spectrum was 62.86% and 67.57% respectively.Conclusion It is successful to find serum different expression proteins in patients with cyst echinococcosis by SELDI-TOF-MS and Bio-informatics.It would be a diagnosis method or research method of cyst echinococcosis.
