62例亲属活体供者术后围手术期安全的评估

摘　　要：目的探讨亲属活体供者术后围手术期的安全性。方法术前对62例供者的一般状况、重要脏器功能、配型、双肾功能和解剖学进行评估,除外糖尿病、高血压及传染病等。观察术前、术后第1、7天供者的肾功能、术后第1、7天尿常规及发生并发症等情况。结果62例供者术前血肌酐平均为(63.26±13.82)μmol/L,术后第1、7天的血肌酐分别平均为(93.41±25.71)、(85.94±22.63)μmol/L,除3名供者的血肌酐稍高于正常值,余血肌酐均下降至正常范围。62例供者中,术后第1天出现蛋白尿24例,占38.6%;除1例于术后2周时正常外,均于第7天时复查正常。2例术中发生胸膜损伤,经适当处理痊愈。结论亲属活体供肾移植应重视供体安全,术前进行严格的筛选和全面的评估,术中操作规范,术后加强观察和护理,活体供者在围手术期是安全的。Objective To explore the safety of living relative donors in the peri-operation at post-operation.Methods Pre-operation,sixty-two potential donors were evaluated in such aspects as general condition,major organs function,histocompatibility typing,renal function and anatomy,except diabetes,hypertension and infectious diseases.The results of renal function and the surgical complications of donors were evaluated at pre-operation and post-operation on the first and seventh day.At pre-operation and post-operation.Results 62 donors were identified on the basis of the criteria.Most donors had normal Scr levels,which had a significant increase on the first day of post-operation （93.41± 25.71）μmol/L vs.that pre-operation （63.26±13.82）μmol/L and declined to normal levels （ 85.94± 22.63）μmol/L on the seventh day of post-operation,with the exception of three donors who were found a slight increase of serum creatinine （Scr） exceeding normal range on the first day of post-operation.Proteinuria was observed in 24 cases （ 38.6% ） of 62 cases and the donors recovered on the seventh day.However,there were one donor who recovered in the second week.Two cases of pleural injury were reported and were cured with proper treatment.Conclusion The safety of living donors should be emphasized.It is important that both rigorous selection and preliminary assessment should be performed.Surgical operation is critical for the peri-operation success of living relative kidney transplantation.
