摘 要:目的探讨生物反馈方法对隐性脊椎裂患儿排便功能的影响。方法选取68例隐性脊椎裂合并功能性便秘的患儿进行常规保守治疗。保守治疗无效的患儿接受生物反馈治疗,并进行客观指标检查,评价治疗效果。57例无肛肠及神经系统疾病儿童为正常对照组。结果68例经保守治疗,27例治愈,6例明显好转。35例保守治疗无效的患儿有31例接受全部疗程生物反馈治疗。客观评价指标显示,保守治疗无效患儿反映肛门直肠功能的指标改变明显严重于保守治疗有效的患儿,静息向量容积、收缩向量容积、直肠感觉阈值、静息肌电振幅、会阴一肛门反射潜伏期、传输指数两组比较差异均有显著性(P〈0.05)。生物反馈治疗后,除静息向量容积,其余各项指标与治疗前比较差异有显著性(P〈0.05),其中收缩向量容积、直肠感觉阔值、静息肌电振幅和收缩肌电振幅与正常组比较差异无显著性。基本达到正常。结论隐性脊椎裂可能是功能性便秘的主要原因之一,生物反馈训练是有效的治疗方法。Objective To explore the effect of biofeedback therapy on dysporia in spina bifida occulta (SBO)children with functional constipation(FC). Methods Sixty-eight SBO children with FC under- went conventional trealanent, including oral laxatives, high-fibre diet, and toilet training. Among those children who were unresponsive to conventional treatment, thirty-five children were treated by biofeedback. Results Of the 68 children who received conventional treatment for at least two months ,27 children recovered according to the clinical recovery standard, and 6 improved significantly. Thirty-one children who received biofeedback training reported symptomatic improvements. Comparing to the children who recovered after the conventional treatment,the children who received the biofeedback training had significantly more de- ranged anorectal function, especially resting vector volume, squeezing vector volumes, rectal sensory thresh- old,resting EMG amplitude,transit index and latency of pudendo-anal reflex( P 〈 0. 05 ). After biofeedback therapy, all the indexes showed significant improvements except resting vector volume ( P 〈 0. 05 ). Conclusion SBO is one of major causes in SBO children with FC. The biofeedback training is suitable for improvement of the clinical outcome and physiological measurements.
