摘  要:目的调查辽宁省阜新县农村地区成年人高血压前期进展为高血压的相关预测因素。方法采用分层整群抽样对2004—2008年辽宁省阜新县7个乡镇年龄≥35岁无心血管疾病的高血压前期人群15061人进行随访,由培训过的医师对调查对象进行血压测量及资料的收集。结果在28个月的随访期间有4917名(32.6%)高血压前期人群发展为高血压,然而仅有37例占1.5%的高血压病患者的血压得到控制。Cox逐步回归分析显示:年龄、蒙古族、饮酒、超重和肥胖、每天的食盐摄入量、不适当的体力活动、有高血压家族史与高血压事件的发生相关。结论高血压前期的发病率很高,存在多种危险因素,应积极开展宣教工作,通过改变生活方式,如戒酒,健康的饮食习惯,适当的体力活动,以预防高血压和心脑血管疾病的发生。Objective To investigate the predictors of progression from prehypertension to hypertension among rural Chinese adults in the countryside of Fuxin, Liaoning province. Methods A population-based sample of 15 061 rural Chinese adults aged ≥35 years with prehypertension, free of cardiovascular diseases at baseline were followed up from 2004 to 2008. The blood pressure and correlative risk factors were investigated by trained doctors. Results During the 28-month follow-up period, 4 917 subjects (32.6%) with prehypertension developed hypertension, in which only 1.5 % (n= 37) of incident cases taking blood pressure control. After Cox stepwise regression analysis, baseline age, Mongolian, alcohol drinking, overweight and obesity, salt intake one day, inappropriate physical activity, and family history of hypertension were associated with incident hypertension independent of initial SBP and DBP. Conclusion The results indicated that prehypertension was highly prevalent in Liaoning province; it was associated with many risk factors. Lifestyle modifications such as cessation of drinking, healthy diet, and moderate physical activity are advised to prevent or delay the progression of prehypertension to hypertension, so as to prevent these people from hypertension and cerebrovascular disease in rural area of China.
