舒洛地特和前列腺素E_1联合治疗早期糖尿病肾病的疗效观察

摘　　要：目的探讨舒洛地特、前列腺素E1及二者联合治疗对早期糖尿病肾病的疗效。方法检测96例早期糖尿病肾病(DN)患者的一般临床指标和舒洛地特、前列腺素E1及二者联合治疗1个月、3个月时的尿白蛋白排泄率(UAER)的变化。结果血压、血糖、血红蛋白、肾功能治疗前后比较差异无统计学意义(P>0.05);前列腺素E1、舒洛地特治疗组治疗3个月后纤维蛋白原(Fib)水平明显下降(P<0.05),二者联合治疗组Fib水平下降更显著(P<0.01)。各组治疗过程中UAER的比较:治疗1个月时与治疗前相比3组水平均有所下降(P<0.05),其中联合治疗与单药相比下降更加明显(P<0.05);治疗3个月时较1个月时3组水平有所下降(P<0.05),而联合治疗组与单药治疗组相比下降更加明显(P<0.05)。结论舒洛地特治疗早期DN疗效确切,而舒洛地特和前列腺素E1联合治疗效果更佳。Objective To study the effect of sulodexide and prostaglandin E1 in the treatment of early diabetic nephropathy. Methods The changes in general clinical indexes were detected in 96 patients with early diabetic nephropathy and urinary albumin excretion rate was detected before treatment of sulodexide and prostaglandin El and one month, three months after the treatment of them. Results There wasfft any apparent change in blood pressure,blood glucose, haematoglobin Alc and kidney function in three groups compared with those before treatment （P〉0.05） and after three months of treatment of sulodexide, prostaglandin E1; the levels of fibrinogen were decreased （P〈0.05） and were much lower after co-treatment （P〈0.01）. The level of urinary albumin excretion rate was decreased after one month in treatment of prostaglandin E1, sulodexide therapy （P%0.05） and were much lower after co-treatment （P〈0. 01）; the level of urinary albumin excretion rate was markedly decreased after three months treatment of prostaglandin E1 （P〈0.05） and were much lower after sulodexide therapy and co-treatment （P〈0.01） ;the level of urinary albumin excretion rate was markedly decreased after three months treatment of co-treatment than it was in single therapy （P〈0. 05）. Conclusion The therapy effect of sulodexide is confirmed in early diabetic nephropathy, moreover, the effect of coreatment with sulodexide and prostaglandin E1 is better than that of single treatment. The article provides evidences in treatment of early diabetic nephropathy.
