补中益气汤对封闭抗体缺乏的反复自然流产患者生殖免疫调节研究

摘　　要：目的:观察补中益气汤为主方治疗封闭抗体缺乏的母-胎免疫识别低下型反复自然流产(Recurrent Spontaneous Abortion,RSA)患者的临床疗效。方法:以单向混合淋巴细胞反应封闭试验检测封闭抗体,选取封闭抗体缺乏的RSA患者51例,随机分成两组:一组为中药组(治疗组),共22例,给予补中益气汤治疗;另一组为免疫组(对照组),共29例,以配偶淋巴细胞体外诱生免疫疗法治疗,比较治疗后两组患者的封闭抗体各指标上升情况。结果:经治疗后中药组和免疫组的BE-Ab1、BE-Ab2均较治疗前明显上升(分别为P<0.01,P<0.05);但对流式细胞术分析的抗CD3-BE、抗CD4-BE及抗CD8-BE无明显改善作用(P>0.05)。中药组和免疫组的BE-Ab1、BE-Ab2平均值都随着治疗次数的增加而呈逐渐上升趋势。结论:补中益气汤使可使RSA患者的BE-Ab1、BE-Ab2明显上升,并使BE-Ab1、BE-Ab2随着治疗次数的增加而逐渐上升、封闭抗体正常时妊娠成功率较缺乏时明显上升。由本研究得出:补中益气汤和配偶淋巴细胞体外诱生免疫疗法治疗RSA患者的临床疗效相近。Objective：To observe the clinic effect of Buzhong Yiqi Tang in treating recurrent spontaneous abortion （RSA） of insufficient materno- fetal recognition, with insufficient blocking antibodies. Methods：We examined blocking antibodies with One - way mixed lymphocyte culture （MLC） and divided 51 patients with insuflqcient blocking antibodies into two groups randomly： the treatment group and the control group. The treatment group were treated with Chinese herb： Buzhong Yiqi Tang; the control group were immunized with the in - vitro inducing paternal lymphocytes. The up- grade state of BA and the relationship between the level of BA and times of treatment after treatment were observed. Re- su/ts ： The level of BE - AbI and BE - Ab2 was increased significantly （ P 〈 0.01 and P 〈 O. 05 respectively） than before treatment, but it have not obvious improvement on anti - CD3 blocking antibodies, anti - CD4 blocking antibodies and anti - CDs blocking antibodies in peripheral sera of the RSA patients （ P 〉 0.05 ）. The average value of BE - Abl and BE - Ab2 was increased slowly with increase of treatment times. Conclusion： Both the two methods can promote elevation of BE - AbI , BE - Ab2 obviously, and raise the successful rate of pregnancy, make the average value of BE - Abl and BE - Ab2 increase gradually with the increase of treatment times. Form the study we can include that the clinic effect of Buzhong Yiqi Tang was approximation to paternally lymphocyte immuno - therapy in vitro induction.
