血管蒂髂骨瓣移植辅助血管内介入治疗青壮年股骨颈骨折不愈合的临床研究

摘　　要：目的观察带旋股外动脉升支血管蒂髂骨瓣移植辅助血管内介入对青壮年股骨颈骨折不愈合的治疗效果。方法选取2007年4月～2009年6月我院收治的股骨颈骨折不愈合需手术治疗的青壮年患者79例,依据随机的原则分为观察组56例和对照组23例。观察组患者术前应用血管内介入,行带旋股外动脉升支血管蒂髂骨瓣移植治疗;对照组患者单纯行带旋股外动脉升支血管蒂髂骨瓣移植的手术治疗;同时选取同期22例无强烈治疗愿望的股骨颈骨折不愈合患者作为空白对照组,即不采取任何处理措施。随访观察三组治疗的效果。结果空白对照组患者的优良率为40.00%,对照组患者的优良率为68.18%,观察组患者的优良率为94.55%,三组患者的优良率依次升高。观察组中不同性别、不同年龄患者的优良率未见明显差别。结论临床常见的青壮年股骨颈不愈合患者,在介入治疗的基础上行带旋股外动脉升支血管蒂髂骨瓣移植治疗,临床疗效好,适合应用。Objective To observe the therapeutic effect of ascending branch of femoral artery vascular pedicle iliac bone graft-assisted endovascular intervention on nonunion of femoral neck fracture in the young and the middle-aged. Methods We divided 79 cases of nonunion of femoral neck fracture of the young and the middle-aged in our hospital from April 2007 to June 2009 into two groups,observation group（56 cases） and control group（ 23 cases ）. The observation group received interventional therapy plus implantation. The control group received im- plantation alone. We selected 22 cases as the blank control group which received no treatment. Results The excellent and good rates were 40.00% ,68.18% and 94.55% in the blank control group ,control group and observation group,respectively,higher in the observation group than in the control group and blank control group,and the rate was higher in the control group than that in the blank control group. But there was no significant difference in age and sex in the observation group. Conclusion The nonunion of femoral neck fracture is common and the implantation plus interventional therapy has a satisfactory effect in the treatment of the nonunion of femoral neck fracture, and the therapy is worthy of being recommended in the clinical treatment.
