摘  要:目的:探讨应用电化学发光法(ECLA)检测心力衰竭(CHF)患者血浆N-末端脑利钠肽(NT-proBNP)水平的临床意义。方法:电化学发光法测定67例心力衰竭(CHF)患者与正常对照组16例患者的血浆中NT-proBNP水平。结果:心衰患者血浆NT-proBNP水平明显高于正常对照组[(6435.7±8725.4比78.6±54.3)ng/L,P<0.001];以300 ng/L为正常参考值,NT-proBNP诊断心衰的敏感性92.5%(62/67),特异性93.8%(15/16);不同NYHA心功能分级患者的血浆NT-proBNP浓度比较可见两者随心功能级别增加而显著升高。结论:ECLA法检测血浆中NT-proBNP水平可判断患者是否存在心衰可能;NT-proBNP是一项量化的心功能标志物,在心衰程度诊断、治疗过程监测及预后评估有重要意义;同时具有无创伤、快速可靠、完全自动化。Objective: To explore the clinical significance of Electrochemiluminescence(ECLA) in testing the plasma N-terminal pro-brain natriuretic peptide(NT-proBNP) levels in patients with heart failure(CHF).Method: To detect the NT-pro BNP levels with electrochemiluminescence in 67 patients with CHF and normal control group of 16 patients with plasma NT-proBNP levels.Result: The NT-proBNP levels in patients with CHF were significantly higher than that of control group(6435.7±8725.4 vs 78.6 ±54.3ng/L,P0.001);The NT-proBNP levels 300ng/L as the normal reference value could be used to diagnose heart failure with sensitivity 92.5%(62/67),specificity 93.8%(15/16);the plasma concentrations of NT-proBNP levels of patients with heart failure had a positive correlated with New York Heart Association(NYHA).Conclusion: ECLA detection of the NT-proBNP level may determine whether the patients have heart failure;NT-proBNP level is a quantitative index of cardiac function,it has an important meaning in determining the NYHA,monitoring treatment and evaluating prognosis,it also has non-invasive,fast and reliable,fully automation
