不同手术方法治疗高血压脑出血的临床观察

摘　　要：目的:探讨不同手术方法治疗高血压脑出血(HICH)的临床效果。方法:选取2006年1月至2008年1月间在我院住院治疗的HICH患者90例,随机分为治疗组和对照组;治疗组接受微创小骨窗开颅血肿清除术,对照组接受传统大骨瓣开颅血肿清除术,追踪随访临床效果。结果:治疗后治疗组脑水肿A级患者例数明显多于对照组,而D级患者例数明显少于对照组,两者比较差异均具有统计学意义(P<0.05)。治疗后治疗组日常生活能力评定Ⅰ级、Ⅱ级患者例数明显多于对照组,而Ⅲ级、Ⅳ级患者例数明显少于对照组,两者比较差异均具有统计学意义(P<0.05)。治疗后第1、2、4周神经功能缺损评分治疗组明显优于对照组,两者比较差异具有统计学意义(P<0.05)。结论:微创小骨窗清除术疗效明显优于传统开颅术,是高血压性脑出血较理想的术式,值得临床推广应用。Objective： To investigate the clinical effects of different measures in treating patients with hypertensive intracerebral hemorrhage.Method： 90 cases with hypertensive intracerebral hemorrhage from January in 2006 to January in 2008 were divided into the treatment group and control group,and the treatment group received the key-hole surgery and the control group received the large trauma craniotomy.All of patients were followed-up.Result： The case with A grade cerebral edema in treatment group after treatment were more than those in control group,the case with D grade cerebral edema in treatment group were fewer than those in control group,and there was significant difference between two groups（P0.05）.The case with Ⅰand Ⅱgrade of daily living activities in treatment group after treatment were more than those in control group,the case with Ⅲ and Ⅳ grade of daily living activities in treatment group were fewer than those in control group,and there was significant difference between two groups（P0.05）.The neurologic impairment evaluation in treatment group after treatment in 1,2 and 4 weeks were superior to those in control group,and there was significant difference between two groups（P0.05）.Conclusion： The key-hole surgery is superior to the traditional operation,and is the ideal operation for patients with hypertensive intracerebral hemorrhage,so it is worth being used widely.
