自体疝囊填充式无张力疝修补术的临床应用

摘　　要：目的:探讨无张力疝修补术在基层医院的临床应用。方法:回顾性分析2007年5月～2008年5月云南省祿劝县第一人民医院外一科的43例原发腹股沟疝自体疝囊填充式无张力疝修补术的临床资料。结果:手术时间40～70min,平均50min,术后12h下床,最短术后6h下床,伤口红肿4例,尿潴留2例。术后2～3周多可进行适当体力劳动。随访最长2年,无一例复发。结论:无张力疝修补术手术指征宽、恢复快、术后疼痛轻、复发率低。用自体疝囊替代人工材料的疝填充物,疗效与疝环填充式无张力疝修补术相似,费用明显减少。Objective To investigate the clinical application of tension-free hernia repair in basic-level hospitals.Method The clinical data of 43 patients with primary inguinal hernia treated by autologous hernial sac filled tension-free hernia repair were retrospective analyzed.Results The operation time was 40 ～ 70 min,the average was 50 min,out-of-bed activity was at 12 h after operation,and the shortest was 6 h postoperation,4 cases with incision red swelling,and 2 cases with urinary retention.The patients could have proper physical labor 2-3 weeks after operation.After 2 years′ follow-up,no recurrence was found.Conclusion Tension-free hernia repair has wide surgical indications,rapid recovery,less postoperative pain,and low recurrence rate.Using autologous hernial sac replace hernia padding with artificial material,the curative effect is similar to tension-free hernia ring filled hernia repair,and the expenses obviously reduced.
