高血压患者血清脂联素水平变化及相关因素分析

摘　　要：目的探讨高血压(EH)患者血清脂联素(APN)水平变化及其与肾素-血管紧张素系统(RAS)的关系。方法选择单纯EH患者33例,EH合并代谢综合征(MS)患者42例,健康对照者30例,检测每位受试者血清APN、血管紧张素Ⅱ(AgⅡ),并收集MS的相关指标,比较三组间APN水平的差异及其与AgⅡ的关系。结果(1)单纯EH组和EH+MS组的APN水平明显低于健康对照组,EH+MS组降低更明显(P<0.05);而单纯EH组和EH+MS组AgⅡ浓度明显高于健康对照组,EH+MS组的升高更明显(P<0.05);(2)随着血压水平的升高,APN水平逐渐降低,AgⅡ升高(P<0.05);(3)在EH组中,APN与AgⅡ成负相关,AgⅡ是影响APN水平的重要危险因素之一。结论EH患者APN水平降低,合并MS的EH患者APN水平进一步降低,AgⅡ、高密度脂蛋白胆固醇(HDL-C)和血压是影响APN水平的重要因素。Objective To explore the study on the diversity of serum adiponectin（APN） level in patients with essential hypertension and the correlation between APN and rennin-angiotensin.Methods 33 simple essential hypertension patients without incorporated metabolic syndrome（EH）,42 essential hypertension incorporated metabolic syndrome patients（EH＋MS） and 30 normal adults were selected.APN level,angiotensin-Ⅱ（AgⅡ） were used to assess alterations associated with hypertension patients without incorporated metabolic syndrome,essential hypertension incorporated metabolic syndrome patients and normal controls involved in its effect on hypertension,as well as to explore the correlation between APN and AgⅡ.Results EH＋MS group had lower APN level and higher AgⅡ level than those in other two groups,and the difference of the three interclass reached statistical significance（P0.05）.The APN level decreased,while AgⅡ level gradually increased with elevated blood pressure（P0.05）.In the hypertension group,the APN level were negatively correlated with AgⅡ（P0.05）,AgⅡ was one of the significant risk factors to influence the level of APN.Conclusion Essential hypertension patients have low APN level,EH＋MS group assembled multiple cardiovascular dangerous factors,so have an obviously lower APN level.Furthermore,AgⅡ,HDL-C and blood pressure is the significance risk factors to affect APN.
