摘 要:目的探讨青藤碱(sinomenine,Sin)对缺血再灌注损伤大鼠脑组织保护作用及对环氧化酶-2(cyclooxygenase-2,COX-2)表达和血脑屏障通透性的影响。方法将大鼠随机分为假手术组、缺血再灌注组、Sin低剂量治疗组和Sin高剂量治疗组,线栓法建立局灶性脑缺血再灌注模型。Sin低(Sin30 mg/kg)、高剂量(60 mg/kg)治疗组于术前30 min分别给予大鼠腹腔注射。用免疫组化法观察缺血90 min再灌注24 h大鼠额顶部皮质COX-2表达及脑含水量和伊文斯蓝(EB)含量变化,并进行2,3,5-三苯基氯化四氮唑(TTC)染色观察脑梗塞体积变化和神经症状评分。结果Sin高、低剂量治疗组神经功能缺失评分较缺血再灌注组降低,高、低剂量组之间差异亦具有统计学意义(P<0.05)。Sin高、低剂量治疗组脑梗塞体积较缺血再灌注组减小,高、低剂量组之间差异亦具有统计学意义(P<0.05)。与假手术组比较,缺血再灌注组额顶部皮质COX-2表达明显增加;与缺血再灌注组比较,Sin高、低剂量治疗组COX-2表达均显著减少(P<0.05),高、低剂量组之间差异亦具有统计学意义(P<0.05)。与假手术组比较,缺血再灌注组脑含水量和EB含量明显升高;与缺血再灌注组比较,Sin高、低剂量治疗组脑含水量和EB含量明显降低(P<0.05),高、低剂量组之间差异亦具有统计学意义(P<0.05)。结论Sin对缺血再灌注脑损伤具有保护作用,其机制可能是通过抑制再灌注损伤后COX-2表达,减轻血脑屏障通透性。Objective To study the the effect of sinomenine on expression of cyclooxygenase - 2 and the permeability of the blood - brain barrier of cerebral ischemia reperfusion(I/R) injury in rats. Methods In this experiment, rats were randomly divided into 4 groups:sham operated group, I/R group, low sinomenine treated group and high sinomenine treated group. The focal middle cerebral artery occlusion(MCAO) model was made by suture -occluded method. Sinomenine was given intraperitoneally to rats 30min before focal cerebral ischemia operation respectively . After 90min MCAO following 24h of reperfusion, the expression of eyclooxygenase - 2 with immunohistochemistry were investigated. Brain water content and the content of Evens blue (EB) were investigated . TTC staining and the neurological score were also investigated. Results Compared with IR group, the neurologieal score was decreased in low and high dose sinomenine treated groups ( all P 〈 0.05 ). ( 2 ) Compared with I/R group, low and high sinomenine treated groups dose - dependently reduced cerebral infarction ( all P 〈 0.05 ). ( 3 ) Compared with sham operated group , expression of cyelooxygenase -2 was increased at 24h reperfusion in the frontal and parietal cortex (P 〈 0.05 ). Compared with I/R group, low and high sinomenine treated groups dose - dependently reduced expression of cyelooxygenase - 2 ( all P 〈 0. 05 ). (4) Compared with sham operated group, brain water content and the content of EB were increased at 24h of reperfusion. Compared with I/R groups, low and high sinomenine treated group dose - dependently reduced brain water content and the content of EB (P 〈 0.05 ). Conclusion Sinomenine may obviously reduce cerebral isehemia -reperfusion injury by deereasing the expression of cyclooxygenase - 2 and the permeability of blood - brain barrier . It has protective effect on cerebral ischemia injury.
