摘 要:目的探讨沙利度胺(Thd)对系统性红斑狼疮(SLE)患者外周血T淋巴细胞的免疫调节作用。方法用四甲基偶氮唑蓝(MTT)比色法检测Thd对SLE患者外周血T淋巴细胞增殖的影响,用流式细胞术检测T淋巴细胞早期凋亡及CD3+CD28+和CD8+CD152+的表达,用实时定量聚合酶链反应(PCR)检测T淋巴细胞白细胞介素(IL)-6、IL-10、肿瘤坏死因子(TNF)-α mRNA的表达,采用单因素方差分析进行统计学处理。结果在体外,500ug/ml Thd组CD3+CD28+表达为(48+9)%显著低于对照组(57±9)%(P〈0.05)、500p.g/mlThd组T淋巴细胞凋亡率为(36+8)%显著高于对照组(23±5)%(P〈0.05);100、300、500拉g/mlThd组A570nm值分别为:0.39±0.05、0.34+0.04、0.30±0.03较对照组(0.51±0.07)均有明显下降(P〈0.05);100、500ug/mlThd组CD8+CD152+表达分别为(5.0±0.6)%、(7.8±0.7)%,明显高于对照组(4.2±0.6)%(P〈0.05);500ug/ml Thd能抑制IL-6mRNA的表达,各剂量组均抑制IL-10、TNF-α mRNA表达。结论Thd可能通过抑制T淋巴细胞增殖、CD28的表达、IL-6、IL-10、TNF-α mRNA的表达和促进T淋巴细胞凋亡、CD152表达来下调SLE患者的免疫反应。Objective To investigate the immunoregulatory effect of thalidomide on the peripheral blood T-lymphocytes in systemic lupus erythematosus patients in vitro. Methods T-lymphocytes were treated by thalidomide with different concentrations, then the proliferation of these T-lymphocytes proliferation was deteted by MTE while apoptosis and lymphocyte activation marker were analyzed by flow eytometry. The mRNA expression of IL-6, IL-10 and TNF-a was measured by real-time RT-PCR, One-way ANOVA was used for statistical analysis. Results In vitro, thalidomide inhibited the expression of CD3+CD28+[500 ug/ml group vs the control group, (48±9)% vs (57±9)% P〈0.05 ]. The pro-portion of apoptotic T-lymphoeytes in the 500 ug/ml group was (36+8)%, which was significantly higher than that in the control group [ (23±5)%, P〈0.05 ]. The values of A 570nm T-lymphocytes were significantly lower in the 100 ug/ml group, 300 ug/ml group and 500 ug/ml group compared with those of the control group ( 100 ug/ml group vs 300 ug/ml group vs 500ug/ml group vs the control group, 0.39±0.05 vs 0.34±0.04 vs 0.30±0.03 vs 0.51 ±0.07, P〈0.05), while thalidomide promoted the expression of CD8+CD152+ [ 100 ug/ml group vs 500 ug/ml group vs the control group, (5.0±0.6)% vs (7.8±0.7)% vs (4.2±0.6)%, P〈0.05 ]. 500 ug/ml thalidomide inhibited the mRNA expression of IL-6, 2.5-500ug/ml thalidomide inhibited IL-10, TNF-α mRNA expression of T-lymphoeytes. Conclusion Thalidomide can inhibit the proliferative activities and CD28 expression of T-lymphoeytes, reduce mRNA expression of IL-6, IL-10, TNF-α, stimulate CD28 expression and apoptosis of T-lymphocytes. These effects may play an important role in it's immune-suppressive effects on systemic lupus erythematosus.
