前列腺癌骨转移患者血清ICTP、PINP的表达及临床价值

摘　　要：目的探讨血清ICTP、PINP在前列腺癌骨转移患者中的表达及临床价值。方法根据骨显像将62例前列腺癌患者分为对照组(无骨转移组)28例和实验组(骨转移组)34例,采用Soloway分级标准,根据骨转移灶个数将实验组再分为A、B、C三个亚组。采用酶联免疫检测法(ELISA)检测实验组和对照组血清型胶原羧基端肽(ICTP)、型前胶原氨基端前肽(PINP)蛋白水平。检测血清ICTP、PINP在A、B、C三个亚组的变化情况,分析ICTP、PINP与骨转移灶个数的相关性。结果实验组血清ICTP、PINP较对照组升高,差异均有统计学意义(P<0.01);血清ICTP、PINP在A、B、C三组总体有依次递增趋势,组间比较差异有统计学意义(P<0.05);血清ICTP、PINP与骨转移灶数目呈正相关,相关系数分别为0.51和0.47(P<0.05);血清ICTP与PINP呈中度相关,相关系数为0.58(P<0.01)。结论血清ICTP、PINP可能为诊断前列腺癌骨转移的良好指标。Objective To study the expression and clinical value of serum carboxyterminal telopeptide of type I collagen（ICTP） and procollagen I N-terminal propeptide（PINP） in prostatic cancer patients with bone metastases.Methods Serum ICTP and PINP contents were measured with ELISA in 28 patients without bone metastases（control group） and 34 with bone metastases（experimental group） according to bone imaging.The experimental group was then divided into A,B,C subgroups according to Soloway classification standard.The comparison of bone turnovers among patients with different bone loads and the relationship between ICTP and PINP were analyzed.Results Serum ICTP and PINP in experimental group were significantly higher than control group（P〈0.01）.With the increase of the extent of bone metastases,ICTP and PINP elevated,there were significant differences between groups（P〈0.05）.Levels of ICTP and PINP correlated well to the number of foci（r=0.51,P〈0.05,r=0.47,P〈0.05）.There was midrange correlation between serum ICTP and PINP（r=0.58,P〈0.01）.Conclusion Serum ICTP and PINP may be good markers in the diagnosis of bone metastases in prostatic cancer patients.
