血浆同型半胱氨酸与原发性高血压关系的研究

摘　　要：目的 探讨原发性高血压患者血浆同型半胱氨酸（homocysteine，HCY）水平的变化情况与高血压不同危险分层、性别等的相关性。方法选择76例50-75岁之间排除严重疾病，严重并发症门诊的高血压患者，按《中国高血压防治指南》（2005年修订版）的危险分层标准，分为2个组：Ⅰ组（中危组）、Ⅱ组（高危组），并设正常对照组。分别检测其外周血HCY浓度并进行组间、以及同一组间性别的比较。结果和正常对照组相比随着危险程度的增高，血HCY的浓度也逐渐升高，Ⅰ组与Ⅱ组以及Ⅰ组与Ⅱ组不同性别间HCY水平间差异均有统计学意义（P〈0．05）。说明血HCY浓度与高血压的危险程度呈正相关．同时同一高血压危险分层中男性血HCY浓度增高比女性血HCY浓度增高明显。结论高血压患者血浆同型半胱氨酸水平与高血压患者的危险分层及性别密切相关，血浆同型半胱氨酸值有助于判断原发性高血压患者发生心、脑、血管疾病的危险性．对高血压的预防、诊断和治疗具有一定的临床指导意义。Objective To explore the correlation among plasma homocysteine （Hcy） content and the different risk stratification, sexin essential hypertensive patients. Methods According to 《The Guidelines on Prevention and Management of Hypertension in China》（Revised edition in 2005） , the 100 patients exclusing serious diseases and serious complications among 50-75 age old with hypertension in out-patient clinic（OPD） were divided into two groups： I group （medium risk group ）, Ⅱ group （ high risk group ） , then we measured the content of HCY in peripheral blood and compared between groups, sex. Results The content of HCY is increasing with the risk stratification compared to normal control group. A statistical significance occurs on the essential hypertensive patients with medium - high risk and difference sex in medium - high risk （ P 〈 0.05） ; the plasma HCY had a positive correlation with the risk stratification in hypertension. Conclusion The content of plasma HCY is closing associated with the risk stratification and sex in hypertension. The level of plasma HCY can help us to assess the risk of cardiology,cerebral and vascular accidents and may has some clinical significance in the prevention ,diagnosis and treatment of hypertension.
