摘  要:目的:运用多层螺旋CT研究评价肺血管与脓毒性肺栓塞滋养血管征之间关系。方法:采用双盲法由两名放射科医师回顾性分析11例脓毒性肺栓塞患者MSCT图像,通过横断面、多平面重建(MPR)、最大密度投影(MIP)分析肺结节与肺血管关系,滋养血管征定义为一支血管通向肺结节。结果:脓毒性肺栓塞患者发现65个结节和20个楔形阴影。其中滋养血管征在横断位图像显示26(40%)个结节和5(25%)个楔形阴影与血管连接,而在MPR和MIP显示这些血管绕行结节周围。在所有的成像中,12(18%)个结节和2(10%)个楔形阴影显示与血管连接。在横断位像所有这些血管汇入左心房与肺静脉一致。结论:脓毒性肺栓塞在横断位图像经常能显示滋养血管,但是MPR能显示大多数滋养血管环绕结节走行,"滋养血管"在病例表现为肺静脉。Objective:To study the relationship between pulmonary vessels and feeding vessel sign in septic pulmonary embolism using multi-slice spiral CT.Methods:Two radiologists had a retrospective analysis of MSCT images of 11 patients who suffered from septic pulmonary embolism by double blind method.Cross-sectional images,multiplanar reconstructions,and maximum intensity projections were used to analyze the relationship between pulmonary nodules and pulmonary vessels.The feeding vessel sign was a vessel which led to a pulmonary nodule.Results:65 nodules and 20 wedge-shape opacities were found in all images of patients with septic pulmonary embolism.Among all feeding vessel signs,vessels were found to connect nodules in 26(40%) nodules and 5(25%) wedge-shape opacities on cross-sectional images.But,multiplanar reconstructions and maximum intensity projections showed those vessels surrounded nodules.In all imaging planes,12(18%) nodules and 2(10%) wedge-shape opacities showed that a central vessel connected the focus of septic emboli.In cross-sectional images,all the vessels connected left atrium in accordance with pulmonary vein branches.Conclusion:Septic pulmonary embolism often shows to have the feeding vessels on transverse images that most of these vessels course around the nodule on multiplanar reconstructions,and feeding vessels appear to be images of pulmonary veins branches in those cases.
