摘  要:目的了解及探讨噪声暴露工人出现的主要临床表现及实验室早期较为灵敏的观察指标.方法选择某纺织厂毛巾车间与床单车间接触稳态噪声大于国家职业卫生标准限值85dB(A)工人189名,同时选择接触噪声低于85dB(A)某百货公司商店营业员45名为对照组,详细询问工人及营业员噪声所致的常见临床表现并测定尿中尿香草扁桃酸(VMA).结果观察到接触噪声组工人耳鸣、头晕、多梦、乏力、记忆力减退、肢体麻木等神经衰弱症状明显高于对照组,两组间差异有统计学意义(P<0.05),接触噪声组工人尿中VMA含量明显高于对照组,两组间有统计学意义(P<0.01).结论接触超过国家卫生标准噪声的工人的神经及内分泌系统的变化存在一定关系,测定工人尿中VMA可作为在噪声作用下反映交感神经与内分泌功能状态的客观参考指标.Objective To explore and study the main clinical manifestations and early laboratory observation of more sensitive indicators of the workers exposed to noise.Methods 189 textile mill workers of towels workshop and linens workshop were selected as noise exposed group who were exposed to stable noise more than the limits of national occupational health standards for 85 dB(A),while 45 shop assistants from a department store who were exposed to noise less than 85 dB(A) as the control group.Subjects in the two groups were asked in detail about common clinical manifestations caused by noise and the levels of Vanillylmandelic acid(VMA) in urine were measured respectively.Results The incidence of tinnitus,dizziness,dreaminess,fatigue,hypomnesia,limbs numbness and other neurasthenia-like symptoms in the noise exposed group were significantly higher than those of the control group(P0.05).The levels of urine VMA in the noise exposed group were significantly higher than those of the control group(P0.01).Conclusions These results suggest that there are adverse effects of nervous and endocrine system due to the exposure of higher level noise(≥85 dB(A).The detection of VMA levels in urine can provide reliable experimental evidence for early adverse effects ofsympathetic and endocrine function of the workers exposed to noise.
