摘  要:目的:探讨在低密度脂蛋白胆固醇(LDL-C)水平正常的急性冠脉综合征(ACS)患者联合测定心型脂肪酸结合蛋白(h-FABP),并超敏C-反应蛋白(hs-CRP)的临床意义。方法:测定28例经冠脉造影排除冠心病的住院患者(对照组)和84例经冠状动脉造影确诊为ACS且LDL-C正常的患者(ACS组)的h-FABP,比较两组间的阳性率。同时测定两组hs-CRP水平,比较其差异,ACS患者又被分为不稳定型心绞痛(UAP)、非ST段抬高型心肌梗塞(NSTEMI)、ST段抬高型心肌梗塞(STEMI)三组,比较各组间hs-CRP水平。结果:ACS患者h-FABP阳性率(72.6%)显著高于对照组的(7.1%)P<0.01。ACS患者的hs-CRP水平明显高于对照组[(27.42±12.70)mg/L∶(4.95±3.14)mg/L,P<0.05],且随着病变进一步进展,其水平亦升高,组间差异显著(P均<0.05)。结论:h-FABP联合hs-CRP可提高ACS早期诊断的准确性,同时hs-CRP水平能预测病变的进展程度。Objective: To investigate the clinical significance detected heart fatty-acid-binding protein (h--FABP) and high sensitive C reactive protein (hs--CRP) in acute coronary syndrome (ACS) patients with normal low density lipoprotein (LDL) level. Methods: The h--FABP was measured in 28 patients who were eliminated coronary heart disease by coronary angiography (control group) and 84 patients with acute coronary syndrome (ACS) by coronary angiography but with normal LDL level (observation group), and compared the h-FABP positive rate in two groups. Also the hs-CRP concentrations were measured in two groups, compared their disparities. The ACS patients were divided into three groups including unstable angina (UAP), non-ST elevated myocardial infarction (NSTEMI), ST elevated myocardial infarction (STEMI), compared the hs--CRP concentration in each group. Results: The positive rate of h --FABP in ACS group (72.6%) was more than that in control group (7.1%) , P〈0.05. The level of hs--CRP in ACS group was much higher than that in control group [ (27.42±12.70) mg/L : (4.95±3.14) mg/L, P〈0.05], the severer of pathological change, the higher of concentration of hs--CRP (P〈0.05). Conclusion: Detect h--FABP and hs CRP can improve the accuracy of early diagnosis of ACS, furthermore, the concentration of hs--CRP may predict the degree of pathological change.
