中医综合疗法治疗产褥期抑郁症的临床研究

摘　　要：目的观察中医综合疗法（中药／音乐／心理治疗）治疗对于产褥期抑郁症的疗效，探讨产褥期抑郁症的中医治疗方法。方法对爱丁堡产后抑郁量表（EPDS）评定并诊断的72例产褥期抑郁症患者，按入选先后随机分为西药组与中医综合治疗组（简称中综组），给予西药帕罗西汀（赛乐特）及中医综合治疗（中药柴胡疏肝散颗粒剂／认知心理治疗／音乐治疗），每组36例。统计分析治疗前后的量表分值减低情况。并使用药物不良反应量表评定药物的不良反应。结果接受4周的治疗后，两组之间减分值比较无统计学差异（P〉0．05），各组治疗前后的比较均有减少，中综组分值分别为（16．62±2．26，13．44±1．96，11．00±2．07，8．69±2．03）；西药组分值分别为（16．39±2．42，13．17±2．44，10．75±1．90，7．92±2．16），治疗前后差异有统计学意义（P〈0．01）；同时发现西药组发生药物不良反应共11例，明显多于中综组的2例（X^2=7．604，P〈0．01）。结论在产褥期抑郁症中，中医综合治疗的疗效与帕罗西汀的疗效相当，并且药物不良反应小，依从性好。在有条件的医疗机构，对于产褥期抑郁症的治疗中可考虑使用中医综合疗法。Objective To observe the therapeutic efficacy and safety of traditional Chinese medicine in the patients with postpartum depression. Methods Seventy-two patients were randomly assigned to receive Paroxetine 20 mg daily （ n = 36） and Traditional Chinese medicine（TCM） （ soup of chaihu shugan, therapy of music and cognize psychology（ n = 36） for up to 4 weeks. Therapeutic efficacy was assessed with the Edinburgh Postnatal Depression Scale （EPDS）. Safety profile was assessed with the Treatment Emergent Symptom Scale （TESS）. Results The therapeutic efficiency in treating the postnatal depression was not statistically different between TCM and paroxetine （P 〉0.05）, In TCM group,the score of 0/1/2/4 weeks was 16.62 ±2.26,13.44 ± 1.96,11.00±2.07,8.69 ±2.03 ; in paroxetine group, it was 16.39 ± 2.42,13.17 ± 2.44,10.75 ± 1.90,7.92± 2.16. There are statistically different after treatment in two groups（ P 〈 0.01 ）. The side effects was statistically different between TCM group（2 cases） and paroetine group（ll cases） （X^2 =7. 604,P 〈0.01）. Condusions The therapeutic efficiency in treating the postnatal depression is not different between TCM and paroxetine, but the side effects of TCM is less than paroxetine.
