摘 要:目的探讨心率变化对多层螺旋CT(MSCT)冠状动脉成像图像质量的影响。方法对300例临床疑似冠心病患者进行平扫及增强扫描。总结不同心率及其变化对冠脉图像质量影响,并观察不同心率组的最佳时相分布规律。结果增强扫描时平均心率为64次/min。共评价冠状动脉3032节段,7组中(≤55、56~60、61~65、66~70、71~75、76~80和≥81次/min)不能评价(血管运动伪影、血管纤细)冠脉节段占冠脉总数百分比分别为:13.4%(70/522)、13.0%(91/702)、19.5%(122/626)、18.6%(82/440)、28.1%(74/263)、24.9%(64/257)和37.8%(84/222)。平均心率、节律变化大与运动伪影冠脉节段数经χ2检验,图像质量差异有统计学意义(P=0.000)。结论在临床应用中,心率快、心律不齐是影响图像质量的重要因素之一。[Objective] To investigate the effect of heart rate variation on image quality of multi-slice spiral CT(MSCT) coronary angiography.[Methods] Plain and enhanced dual-scan computed tomography were performed on 300 patients with clinically suspected coronary heart disease. According the different heart rate and the variation, the imaging quality was summarized, and the optimum phase distribution of different heart rate groups was observed. [Results]The average heart rate of the enhanced scan was 64bpm. 3032 of coronary segments were evaluated. In the 7 groups (≤55 bpm, 56~60 bpm, 61~65bpm, 66~70bpm, 71~75bpm, 76~80bpm and ≥81 bpm), the proportion of segments that could not be assessed due to motion artifact and miniature blood vessel were: 13.4%(70/522), 13.0%(91/702),19.5%(122/626),18.6%(82/440), 28.1%(74/263), 24.9%(64/257) and 37.8%(84/222) respectively. Through χ2 test on average heart rate, rapid change of heart rhythm and the number of segments due to motion artifact, the difference of image quality was significant (P=0.000).[Conclusion]Rapid heart rate, arrhythmia is one of the major sources that affecting the image quality.
