精子形态学、精子-透明质结合试验与体外受精率的相关性研究

摘　　要：目的探讨精子形态学、精子-透明质结合试验(hyaluronan binding assay,HBA)与体外受精率的相关性。方法对体外受精-胚胎移植(in vitro fertilization-embryo transportation,IVF-ET)中精子形态学155例和精子-透明质结合试验38例进行分析,分别观察女方MⅡ卵子1509个和327个,记录受精和未受精卵的数目,并分析精子正常形态率、精子-透明质结合试验结合率和受精率的相关性。结果精子正常形态率≤5%、6%～14%、≥15%对应的MⅡ卵子受精率分别为48.05%、72.25%、81.79%,各组间相比较,差异有显著性(P<0.05)。精子-透明质结合试验的结合率≤40%、41%～69%、≥70%对应的MⅡ卵子受精率分别为55.17%、72.07%、75.31%,≤40%组和其它两组间相比较,差异有显著性(P<0.05),另两组间相比较,差异无显著性(P>0.05)。结论在体外受精过程中,精子正常形态率与卵子的受精率有明显的相关性,而精子-透明质结合试验的结合率与卵子受精率有一定程度的相关性,此两项参数可作为选择不同的辅助生殖技术[体外受精(in vitro fertilization,IVF)或胞浆内单精子显微注射术(intracytoplasmic sperm injec-tion,ICSI)]的依据。Objective To study the correlation between sperm morphology,human sperm-hyaluronan binding assay（HBA）and fertilization rate in vitro. Methods The data of sperm morphology in 155 cases and human sperm-hyaluronan binding assay in 38 cases was analyzed and the fertilization rates for 1 509 and 327 MⅡoocytes were observed respectively during in vitro fertilization-embryo transportation（IVF-ET）,thus to analyze the correlation between normal sperm morphological rate,human sperm-hyaluronan binding rate and fertilization rate. Results The fertilization rates were 48.05%,72.25% and 81.79% in terms of normal sperm morphological rate ≤5%,6%～14% and ≥15% respectively and the difference was significant among three groups（P0.05）. The fertilization rates were 55.17%,72.07% and 75.31% in terms of human sperm-hyaluronan binding rate ≤40%,41%～69% and ≥70% respectively,the difference was significant between ≤40% group and other two groups（P0.05） and there was no significant difference between the other two groups（P0.05）. Conclusion The normal sperm morphological rate was obviously relative to fertilization rate,and sperm-hyaluronan binding rate was relative to fertilization rate at a degree. The sperm morphology and HBA may be two important parameters to evaluate the function of fertilization（IVF or ICSI）.
