米非司酮对孕7~9周人胎盘绒毛滋养细胞VEGF的影响

摘  要:目的通过对比观察米非司酮对孕7~9周人胎盘绒毛滋养细胞血管内皮生长因子(VEGF)表达的影响,探讨米非司酮应用于人孕7~9周药物流产时的终孕机理,为临床上合理扩大米非司酮药流应用范围提供理论基础和实验参考依据。方法采用免疫组化法检测VEGF在米非司酮药流组与人流对照组(孕7~9周各12例)胎盘绒毛中表达的差异,了解米非司酮对孕7~9周人胎盘绒毛VEGF表达的影响。结果 VEGF主要表达于绒毛的滋养细胞和血管内皮细胞胞浆中,米非司酮药流组阳性表达较人流对照组弱(P<0.01)。结论米非司酮作为孕激素受体拮抗剂,应用于人孕7~9周药物流产时,可通过降低VEGF在人早孕期绒毛滋养细胞中的表达,抑制胎盘血管的发育,进而不利于妊娠的维持。Aim To observe the effect of mifepfistone on the expression of vascular endothelial growth factor (VEGF) in human 7-9week gestafional placental villous trophoblast. Methods Immunohistochemistry (streptavidin-peroxidase, S- P)was used to detect the expression of VEGF in human placental villous trophoblast in mifepristone group and induced abortion group. Results VEGF was expressed in the cytoplasm of trophoblast and vascular endothelial cell in above mentioned groups, and the expression in mifepristone group was significantly lower than that of the induced abortion group (P〈0.01). Conclution Mifepfistone as a antiprogestin can induce the expression of VEGF in human 7-9-week gestational placental villous trophoblast and it can inhibit the development of placental blood vessel and affect the gestational process by reducing the expression of VEGF in human genstational villous trophoblast.
