外剥内扎硬注加辅助切口术治疗环状混合痔的临床观察

摘　　要：目的探讨开放性痔外剥内扎硬注加辅助切口的方式治疗环状混合痔的临床效果。方法将我院2000年1月至2008年12月收治的120例环状混合痔,选取具有可比性的观察组(外剥内扎硬注加辅助切口术治疗)和对照组(外切内扎硬注术治疗)各60例,在疗效、疗程、并发症、复发、后遗症等方面作对比分析。结果观察组总有效率100%;平均疗程(12.96±3.61)d;术后并发症1例(出血、水肿),随访2年复发2例。对照组总有效88.33%,平均疗程(16.23±4.49)d;术后并发症(出血、水肿)22例;随访2年复发12例。结论观察组明显优于对照组(P<0.01)。外剥内扎硬注加辅助切口术治疗环状混合痔疗效确切、复发率低、术后并发症少,是治疗环状混合痔较好的方法。Objective To explore a combined therapy of external dissection and internal ligation with posterior incision plus injection in the treatment of circumferential mixed hemorrhoids. Methods Clinical data of 120 cases of circumferential mixed hemorrhoids from Jan. 2000 to Dec. 2008 were retrospectively analyzed. The observation group （n=60） was treated by external dissection and internal ligation with posterior incision plus injection. The control group （n=60） was treated by external dissection and internal ligation plus injection.Results In the observation group the total effective rate was 100%,mean treatment course was 12.96±3.61 days,1 case have post operative complication （bleeding,edema）. The 2-year follow-up showed that there were 2 cases relapse and no anal stricture. In the control group the total effective rate was 88.33%,mean treatment course was 16.23±4.49 days,22cases have postoperative complication （bleeding,edema）. The 2-year follow-up showed that there were 12 cases relapse and 15 anal stricture.Conclusion External dissection and internal ligation with posterior incision plus injection is an effective method with low recurrence rate and fewer postoperative complications in the treatment of circumferential mixed hemorrhoids.
