摘  要:目的通过三维测量胫骨近端截骨后的形态学数据,探讨更适合国人、个性化的假体设计。方法对55位志愿者中的92例正常膝关节(男性42例,女性50例)进行螺旋CT扫描,将断层扫描数据导入Mimics10.1软件建立膝关节三维数字模型,然后再将模型导入Geomagic Studio 8软件系统对胫骨近端模型进行模拟截骨、形态学测量及统计分析,并将获得的数据与目前常用假体的形态学参数进行对比。结果胫骨近端截骨面的前后径(AP)为(49.87±3.9)mm、内外侧横径(ML)为(73.50±5.6)mm,AP与ML呈正相关关系;男性胫骨平台AP为(53.24±4.1)mm、ML为(75.85±3.5)mm,比女性AP(47.61±3.7)mm、ML(67.68±2.6)mm都要大;内侧前后径(MAP)比外侧前后径(LAP)要大,男性大(3.9±2.9)mm,女性大(3.7±2.7)mm。结论不同性别及人种的胫骨截面形态可存在显著差异,在进行国产膝关节胫骨假体设计时需要考虑人种、性别等因素。Objective ToexplorethepreferableprosthesisdesignforChinesepeople,byanalyzingthe normal linear parameters of normal proximal tibia. Methods Original CT image data were used to set up three-dimensional model of normal knees. The related linear parameters of different proximal tibia sections were measured and compared,involving the mediolateral (ML),middle anteroposterior (AP),medial and lateral anteroposterior dimensions and the aspect ratio (AP /ML) of the resected proximal tibial surface. Results Positive correlation appeared between AP and ML on the proximal tibia section,which were (49.87±3.9)mm and (73.50±5.6)mm respectively. The AP and ML of male on the tibial plateau were (53.24±4.1)mm and (75.85±3.5)mm,which was more large than that of female of (47.61±3.7)mm and (67.68±2.6)mm respectively. The MAP was larger than LAP,which were (3.9±2.9)mm,and (3.7±2.7)mm for male and female respectively. Conclusions The geometry and anatomy of the knee is variable,regardless of gender and human race,which should be considered carefully during the design process of the tibia prosthesis for the total knee arthroplasty.
