摘 要:目的:诱导测定哮喘患者痰液ECP、TNF-α、slL-2R水平,评价吸入地塞米松防治哮喘的作用。方法:采用酶联免疫法及荧光免疫法对40例轻中度哮喘患者在吸入激素前后测定痰液中嗜酸细胞阳离子蛋白(ECP)、肿瘤坏死因子-α(TNF-α)、可溶性白细胞介素2受体(slL-2R)水平。结果:吸入激素后诱导痰液ECP水平由300±122μg/L降至146±96μg/L、TNF-α由280±135ng/L降至132±53ng/L、slL-2R由497±136kU/L降至328±97kU/L。哮喘患者吸入激素前ECP与TNF-α,slL-2R均呈显著正相关(r=0.58,0.62,P<0.05)。吸入激素后各指标间无显著相关性。结论:吸入激素后哮喘患者诱导痰液中ECP、TNF-α、slL-2R浓度比吸入激素前明显降低。吸入激素可明显抑制气道炎性反应,其作用机制可能与抗炎作用有关。Objective:To investigate the effects of inhaled Dexamethasone on eosinophil cationic protein(ECP),tumor necrosis factor α(TNF-α) and soluble interleukin 2 receptor(slL-2R)levels in asthmatic patients induced sputum.Methods:The ECP,TNF-α and slL-2R levels in induced sputum in 40 patients with mild-moderate asthma were detected before and after Dexamethasone inhalation by ELISA and fluorescence immunity methods.Results:The levels of ECP,TNF-α and slL-2R were 300±122 μg/L,280±135 ng/L and 497±136 kU/L before Dexamethasone inhalation,and 146±96 μg/L,132±53 ng/L and 328±97 kU/L after Dexamethasone inhalation.Before Dexamethasone inhalation,there were significantly positive correlations between the ECP level and the TNF-α1evels(r=0.58,P0.05) and between the ECP level and the slL-2R levels(r=0.62,P0.05).No relation was found among them after Dexamethasone inhalation.Conclusion:The decrease of the ECP,TNF-α and slL -2 R levels in sputum after Dexamethasone inhalation can inhibit the inflammatory reaction of respiratory passage in asthmatic patients,indicating that Dexamethasone may be helpful to anti-inflammatory effect in asthmatic patients.
