病灶清除内固定结合抗痨一号方治疗腰椎结核临床观察

摘　　要：目的：探讨前路病灶清除加后路植骨内固定,结合中药汤剂抗痨一号方内服治疗腰椎结核。方法：回顾性分析自2003年1月至2008年11月,采用前路病灶清除加植骨内固定治疗腰椎结核患者18例,术后服用抗痨一号方治疗,对其进行总结。结果：本组18例患者无术中并发症,切口均一期愈合。随访8~28个月,平均16个月,结核无复发,植骨全部成活,无侧弯后凸畸形,内固定无松动断裂,所有病人全身状况良好。结论：前路病灶清除、加后路植骨内固定治疗腰椎结核疗效确切,脊柱稳定性重建可靠,矫正侧弯后凸畸形理想,术后服用抗痨一号方伤口愈合快,复发率低,是治疗腰椎结核的有效方法。Objective：To explore the combination of anterior debridement and retrospective grafting plus internal fixation in the treatment of tuberculosis of lumbar spine.Methods：Retrospectively analyse 18 cases of spinal tuberculosis under the targeted treatment and then achieve a specific conclusion according to the collected data.Results：no intraoperative complications;primary wound healing.During the 8-28 months follow-up tracking,no recurrence of tuberculosis of lumbar spine and complete survival of grafted bones could be observed without scoliosis kyphosis nor loosening or fracture of internal fixation.All patients prove in good physical condition.Conclusion： With the reliable reconstruction of spinal stability and the good correcting effect of scoliosis and kyphosis,this combined treatment proves quite effective for tuberculosis of lumbar spine.
