整合子及其相关基因盒在临床产ESBLs肺炎克雷伯菌的分布

摘　　要：目的检测Ⅰ～Ⅲ类整合子及Ⅰ类整合子相关基因盒在产ESBLs肺炎克雷伯菌临床分离株中的分布，分析整合子对细菌耐药性的影响。方法用K—B纸片扩散法对108株产ESBLs肺炎克雷伯菌临床分离株进行药敏试验；采用聚合酶链反应（PCR）分析108株产ESBLs肺炎克雷伯菌株的Ⅰ～Ⅲ类整合子；对Ⅰ类整合子阳性菌进行整合子相关基因盒检测。结果108株菌中有58株（53．70％）含有Ⅰ类整合子，未检测到Ⅱ类整合子及Ⅲ类整合子阳性菌；在Ⅰ类整合子阳性菌中，有52株携带Ⅰ类整合子相关基因盒，占89．66％；分离自同一科室的部分菌株携带大小相同的基因盒；Ⅰ类整合子阳性菌株的耐药率高于整合子阴性的菌株。结论Ⅰ类整合子及整合子相关基因盒在产ESBLs肺炎克雷伯菌临床分离株中分布广泛，整合子在细菌耐药中发挥作用。OBJECTIVE To investigated the distribution and characterization of the 3 classes intergrons （ Ⅰ , Ⅱ , Ⅲ） in order to clarify their influence on the bacterial drug resistance. METHODS Kirby-Bauer test was used for antibiotics susceptibility. A total of 108 clinical isolates of Klebsiella pneumoniae were examined with specific primers of int Ⅰ 1 ,int Ⅰ 2 and int Ⅰ 3 by PCR. RESULTS It was found that among 108 strains of K. penumonie, 58 strains （53.70%） were positive for int I 1, and none was positive for int Ⅰ2 and int Ⅰ 3. Furthermore,among the 58 int Ⅰ 1 positive strains, 52 strains （89. 66%） carried integron int Ⅰ 1 with integrated gene cassettes. The antibiotic resistance of the integron positive strains was higher than that of the integron negative ones. CONCLUSIONS lntegrons are prevalent among K. pneurnoniae, showing definite influence on the development of antibiotic resistance.
