摘　　要：目的探讨反复自然流产(RSA)患者主动免疫治疗后血清中抗人类白细胞抗原抗体(HLA抗体)变化及抗体产生与治疗效果的关系。方法采用酶联免疫法(ELISA法)检测RSA患者主动免疫治疗前后血清中的HLA抗体,采用统计分析方法分析抗体产生率及抗体产生与治疗效果的关系。结果180例接受主动免疫治疗并再次妊娠的RSA患者中,治疗后再次成功妊娠141例(成功率78.33%),失败39例(21.67%)。其中产生HLA抗体的83例中,妊娠成功者63例,失败者20例,成功率75.90%;未产生HLA抗体的97例中,妊娠成功者78例,失败者19例,成功率为80.41%。两者比较P>0.05,无统计学意义。结论主动免疫治疗可使大多数RSA患者再次妊娠成功,并能刺激机体产生HLA抗体,但不能以产生HLA抗体作为评价和预测治疗效果的指标。Objective：To investigate the changes of the antibodies against human leukocyte antigen （HLA antibodies）in serum of the Recurrent spontaneous abortion（RSA） patients after active immunotherapy and investigate the relationship between the antibody production and the treatment effect.Methods：The enzyme-linked immunosorbent assay （ELISA） was used to detect the HLA antibodies in serum of the RSA patients before and after the active immunotherapy.And then the methods of statistical analysis were used to analyze the antibody production rate and assay the relationship between the antibody production and the treatment effect.Results：In the 180 cases of the RSA patients who had accepted the active immunotherapy,there were 141 cases that had been successfully treated （the success rate was 78.33%） ,and 39 cases of failure （21.67%）.In all of the 83 cases that had produced HLA antibodies,there were 63 cases of successful pregnancy ,and 20 cases of failure.The success rate was 75.90%.And in all of the 97 cases that had not produced HLA antibodies,there were 78 cases of successful pregnancy ,and 19 cases of failure.The success rate was 80.41%.There was not marked difference（P0.05） between the two groups.Conclusions：Active immunotherapy could stimulate the body to produce HLA antibodies,but HLA antibody was not an indicator that could be used to evaluate and forecast the treatment effect.
