摘 要:目的研究环氧化酶2(COX-2)的表达与术后粘连发生的关系,探讨选择性COX-2抑制剂塞来昔布在预防术后腹腔粘连中的作用。方法健康SD大鼠50只,随机分为5组(n=10),分别为阳性对照组、透明质酸钠组、塞来昔布小剂量组、塞来昔布大剂量组、阴性对照组。每组于造模术后第8天和第15天随机处死大鼠各5只,肉眼观察评估腹腔粘连程度,并取创伤部位肠管及腹膜Western-blot法检测组织COX-2蛋白含量。结果透明质酸钠组、塞来昔布小剂量组、塞来昔布大剂量组腹腔粘连程度以及创伤部位组织COX-2表达明显减低,与阳性对照组相比差异有统计学意义(P〈0.05)。透明质酸钠组与各塞来昔布组相比差异有统计学意义(P〈0.05)。结论COX-2选择性抑制剂塞来昔布通过降低创伤部位组织COX-2表达能减轻术后腹腔粘连。Objective To study the corelation between the expression of COX-2 and postoperative adhesions, and to determine the effect of COX-2 selective inhibitor, Celecoxib, on postoperative adhesion formation. Methods Fifty SD rats were randomly assigned to five groups each consisting of 10 rats. Study groups were as follows : (A) positive control group, (B) sodiumhyaluronate group, (C) low dose Celecoxib group, (D) high dose Celecoxib group, (E) negative control group. Five rats in each group were treated accordingly for consecutive 8 days and 15 days respectively and sacrificed. After treatment, intra-abdominal adhesions were scored using a standard method. The adhesions tissure and injured peritonaeum were subjected to Western-blotting to detect the expression of COX-2. Results The level of postoperative adhesions and expression of COX-2 of sodiumhyaluronate group, low dose Celecoxib group, high dose Celecoxib group were lower than that of positive control group ( P 〈 0.05 ). Sodiumhyaluronate treatment group was different from the two Celecoxib treated groups ( P 〈 0.05 ). Conclusions Selective COX-2 inhibitor, Celecoxib, provides durable inhibition of intra-abdominal adhesions through an antiangiogenic COX-2 mechanism.
