摘 要:目的应用血流速度向量和流线(VFM)技术,从心腔血液流场变化角度评价早期左室舒张功能减低的临床价值。方法选取30例健康志愿者(正常对照组)和30例早期舒张功能减低患者(病例组)为观察对象,对比研究两组漩涡形成规律及漩涡描述参数。结果病例组与正常对照组漩涡形成规律相似,二者漩涡描述参数比较,舒张早、中、晚期,病例组漩涡横径和纵径均小于对照组;舒张早期病例组较对照组更靠近二尖瓣环;病例组的最大向量速度与漩涡圈数与对照组相反,在舒张早期明显低于舒张晚期,在舒张中期低于等容舒张期,P<0.05。结论 VFM技术通过心腔内血液流场变化反映二尖瓣口血流改变,有可能成为一种无创评价左心室舒张功能减低的新方法。Objective To evaluate left ventricular diastolic dysfunction in the way of intracavitary flow structure and flow dynamics by noninvasive vector flow mapping with Doppler echocardiography. Methods Thirty healthy controls and 30 patients with early diastolic dysfunction were involved. The forming law and the description parameters of the vortex were comparatively studied. Results The law of the vortex formation in patients group were similar to control group, but the parameters of the votex were statistically different. In the period of ED, MD and LD, vortex diameter in patients group was smaller than that in control group. In the period of ED, vortex position was closer to mitral valve annulus, and max flow velocity and density number of vortex ring were lower than those in the period of LD, as well as the period MD comparaed with IVC, in contrast to control group, P〈0.05. Conclusions Vector Flow Mapping can be used as an useful noninvasive way to evaluate the left ventricular diastolic function with flow structure and flow dynamics.
