摘　　要：目的探讨对70岁以上高龄患者进行治疗性ERCP治疗的安全性。方法对2006年~2008年进行治疗性ERCP的患者,选择其中≥70岁的109例作为高龄组,〈60岁的245例作为对照组,比较两组患者的手术成功率、术后并发症及平均住院日情况。结果高龄组ERCP成功率为96.3%,对照组为97.5%,两组间差异无统计学意义（P〉0.05）。高龄组术后并发症共11例（10.1%）,其中ERCP本身并发症9例（8.3%）,1例并发症与ERCP有关（1.8%）,1例死于心源性猝死。对照组术后ERCP本身并发症14例（5.7%）,未发生ERCP相关并发症。两组间ERCP本身并发症发生率无统计学意义（P〉0.05）。术后平均住院日,高龄组为（8.2±2.1）d,对照组为（6.3±1.5）d,两组间差异无统计学意义（P〉0.05）。结论对于70岁以上老年患者治疗性ERCP仍然是安全有效的治疗手段。Objective To evaluate the safety of therapeutic ERCP in patients above 70 years old.Methods The patients given therapeutic ERCP treatment from 2006 to 2008 were selected in this study as follows： 109 patients in the experimental group whose ages were older than 70 years,and 245 paients as the control group whose ages were less than 60 years.The success rates of operation,postoperative complications and average hospitalization days were studied in two groups.Results The success rates of operation in the experimental group and the control group were 96.3% and 97.5%,respectively.There was no significant difference between two groups（P〉0.05）.The cases with postoperative complications in patients above 70 years old were 11（10.1%）.In the 11 cases with postoperative complications,9 cases（8.3%） were the complications of ERCP, 1 case（1.8%） was related to ERCP,and 1 case died of sudden cardiac death（1.8%）.There were 14 cases（5.7%） with complications of ERCP in control group,and no complications related to ERCP occurred.There was no significant difference of the complications of ERCP between two groups（P〉0.05）.And there was no significant difference of the average hospitalization days between two groups（8.2±2.1 vs 6.3±1.5,P〉0.05）.Conclusion Therapeutic ERCP is an effective and safe treatment method in patients above 70 years old.
