摘  要:目的:应用心肌矢量应变力和应变率成像技术(Vector strain imaging,VSI)探讨左向右分流型先天性心脏病(VSD,PDA)患儿手术前后左室壁运动的变化。方法:应用VSI对27例先天性心脏病患儿手术前(P组)、手术后1周内(A组)和对照组(N组)21例年龄相匹配的正常儿童左室壁16个节段的纵向、圆周方向、径向运动进行检测。结果:①P组患者左室壁径向运动速度(9节段/16节段)、圆周方向运动速度(3节段/16节段)、纵向运动速度(7节段/16节段)均高于N组,P<0.05,有统计学差异。②A组患者左室壁径向运动速度(5节段/16节段)、圆周方向运动速度(3节段/16节段)、纵向运动速度(2节段/16节段)均高于N组,P<0.05,有统计学差异。③A组患者左室壁径向运动速度(4节段/16节段)、纵向运动速度(9节段/16节段)较P组明显减低,P<0.05;A组与P组患者左室壁各节段圆周方向运动速度无明显差异,P>0.05。结论:左向右分流型先天性心脏病患儿术后1周内左室壁代偿性增强的运动有所降低,逐渐趋于正常,应用VSI可以从径向、圆周方向、纵向全面分析患儿手术前后左室壁运动的变化。Objective: Use vector strain imaging (VSI) to study the perioperative changes in children with congenital heart disease of left to right shunt, and discuss the efficiency and feasibility of VSI. Methods: Twenty-one healthy children(group N) and 27 children with congenital heart disease (including 16 with ventricular septal defects, 11 with patent ductus arteri- ousus) divided into group P (pre-operative) and group A (1 week after operation) were examined, then motion velocities of the 16 left ventricular wall segments in three directions (radial, length, circle) were calculated. Results: ①Comparing the motion velocity of group P with that of group N: group P velocities were higher than group N velocities (P〈0.05) in radial motion of segment 9/16, circle motion of segment 3/16, length motion of segment 7/16. ②Comparing the motion velocities of group A with that of group N: group A velocities were higher than group N velocities (P〈0.05) in radial motion of segment 5/16, circle motion of segment 3/16, length motion of segment 2/16. ③Comparing the motion velocity of group A with that of group P: group A velocities were much lower than group P velocities(P〈0.05), including radial motion of segment 4/16, length motion of segment 9/16. Circle motion of B0 and B1 have no significant difference(P〉0.05). Conclusion: In congenital heart disease with left to right shunt, motions of left ventricle are reduced to normal level regularly with the hemodynamics changes in the group of 1 week after operation. VSI is able to analyze perioperative changes of left ventricular wall motions in radial, circle, length directions.
