胰腺实性假乳头状瘤临床诊治分析(附69例报告)

摘　　要：目的:总结胰腺实性假乳头状瘤(SPT)的临床特点及其治疗、预后。方法:回顾性分析69例SPT患者的临床表现、手术治疗、术后并发症及预后情况。结果:SPT位于胰头部24例(占34.8%)、胰颈部15例(占20.3%)、胰体尾部30例(占45.0%)。69例患者均行手术切除,无手术死亡。术后药物性肝功能衰竭1例、胰瘘9例、胃潴留3例,均保守治疗治愈。术后病理明显恶性者3例,其中1例术后26个月死于肿瘤脑转移,1例术后1年肝内及腹腔淋巴结复发、带瘤生存14个月,1例术后40个月无肿瘤复发。结论:SPT是一种低度恶性的胰腺肿瘤,预后相对良好,胰瘘是术后的主要并发症。Objective： To summarize the clinical features, surgical treatment and prognosis of solid pseudopapillary tumors（SPT）of pancreas. Methods： The clinical features, surgical treatment, postoperative complications and prognosis of the 69 cases were retrospectively studied. Results： There were 24 cases that the SPT located in the head of pancreas, 15 cases in the neck of pancreas, and 30 cases in the body and tail of pancreas. All the cases were performed operation and with no peri-operative death. And all the postoperative complications were healed by use of medical treatment, including one case of drug-induced hepatic failure, 9 cases of pancreatic fistula and 3 cases of delayed gastric emptying. Obvious malignant SPTs were found in three patients, among which, one died from brain metastases 26 months later after the operation, one was found tumor recurrence in liver and celiac lymph nodes one year later after the operation and is still alive now, and no tumor recurrence has been found in the third one since the operation for 40 months. Conclusions： The surgical resection is good treatment for the SPT, which has satisfying prognosis. The major postoperative complication is pancreatic fistula.
