先天性心脏病与病原体感染的关系

摘　　要：目的探讨先天性心脏病（先心病）与弓形虫（TOX）、风疹病毒（RV）、巨细胞病毒（HCMV）和单纯疱疹病毒（HSV）、人微小病毒（HPVB19）感染的关系。方法实验对象分两组，A组为先心病组80例，B组为风湿性心脏病患者和妊娠高血压综合征引产的死婴共32例。采用聚合酶链反应（PCR）荧光定量方法检测两组心肌组织中的5种病原，比较5种病原体的阳性率。同时对阳性者病原基因进行定量，比较病原体数量上是否有差异。结果先心病组中TOX、RV、HCMV、HSV、HPVB19病原基因阳性检出率分别为16．2％、18．8％、28．8％、7．5％、17．5％，对照组分别为0、O、18．8％、6．2％、0。TOX、RV、HPVB19在两组中检测差异有统计学意义（P〈0．05），HCMV、HSV基因检出率间差异无统计学意义（P〉0．05）。先心病与对照组心肌组织HCMV阳性者定量检测值分别为（5．50±1．30）、（5．30±0．33）copies／ml，HSV阳性者定量检测值分别为（4．51±0．87）、（4．32±0．17）copies／ml，先心病组数值高于对照组，但差异无统计学意义。结论先心病的发生与TOX、RV、HCMV、HSV、HPVB19感染存在密切关系。Objective To investigate the relationship between congenital heart dieases （CHD） and toxoplasma （TOX）, rubella virus （RV）, cytomegalovirus （HCMV） and herpes virus （HSV）, human parvovirus B19 （HPVB19） infections. Methods Five pathogenic genes of heart tissue were tested in eases of CHD and eases of controls coming from rheumatic heart disease and death foetus by using fluorescence quantity polymerase chain reaction （PCR）. The positive ratio was compared. At the same time, the quantity of five pathogenic genes was detected. Results The positive ratio of TOX, RV, HCMV, HSV and HPVB19 in heart tissue of CHD was 16. 2% , 18.8%, 28. 8%, 7.5% , and 17. 5% , respectively, and that in controls was 0, 0, 18. 8% , 6. 2% , 0, respectively. There was significant difference in the positive ratio of TOX, RV and HPVB19 between CHD group and control group （P 〈 0.05 ）. The gene quantity of CMV positive individual in CHD group and control group was （ 5.50 ±1.30 ）, and （ 5.30 ±0. 33） copies/ml respectively. The gene quantity of HSV positive individual in CHD group and control group was （4. 51 ±0. 87）, and （4. 32 ±0. 17） copies/ml respectively. There was no significant difference （ P 〉 0. 05 ）. Conclusion There is close relation between pathogen infection and CHD.
