氟比洛芬酯对腹腔镜胆囊切除术患者术后镇痛的效果及血小板聚集的影响

摘　　要：目的:观察氟比洛芬酯对腹腔镜胆囊切除术患者术后镇痛的效果及血小板聚集的影响。方法:选取腹腔镜胆囊切除术患者30例,ASAI级～Ⅱ级,麻醉方法均采用全凭静脉麻醉,随机分成2组,每组15例。Ⅰ组患者在术毕缝皮时静脉注射生理盐水10ml,Ⅱ组患者注射氟比洛芬酯1.0mg/kg。记录患者术后1、3、6、12、24h的安静时VAS评分和活动后VAS评分,以评价镇痛效果。在术前和静脉注射试验药物后1、3、6、12h抽取患者静脉血,测定1min、5min血小板聚集(PAG)功能。记录患者术后24h内发生的不良反应和使用哌替啶补救镇痛的情况。结果:Ⅱ组患者安静时VAS评分在术后6h、24h显著低于Ⅰ组(P<0.05);Ⅱ组患者活动后VAS评分在术后6h显著低于Ⅰ组(P<0.05);2组患者术前和给药后1h、3h、6h、12h血小板聚集功能均无显著性差异(均P>0.05);2组患者术后发生不良反应和使用哌替啶补救镇痛的情况均无显著性差异(均P>0.05)。结论:对腹腔镜胆囊切除术患者,氟比洛芬酯(1.0mg/kg)术后镇痛效果明显,并对血小板聚集无显著影响。Objective：To observe the analgesic effects of flurbiprofen for the patients undergoing laparoscopic cholecystectomy（LC）and their influence on platelet aggregation.Methods：Thirty patients undergoing LC were randomly assigned into tow groups,with fifteen in each group.All the patients were received totally intravenous anesthesia.The patients of GroupⅠ were injected with normal saline 10ml,while the patients of GroupⅡwith flurbiprofen 1.0mg/kg.The visual analogue scale（VAS）at rest and after movement were recorded at 1,3,6,12h postoperatively in order to assess analgesic effect.Their intravenous blood was sampled preoperatively and postoperatively 1,3,6,12 hours in order to measure platelet aggregation of 1min,5min.The side effects and usage of extra analgesic,meperidine within 24 hours postoperatively were recorded.Results：VAS at rest：GroupⅡ was significantly lower than GroupⅠ at 6,24h（P0.05）;VAS after movement：GroupⅡ was significantly lower than GroupⅠ at 6h（P0.05）.The platelet aggregation preoperatively and 1,3,6,12h postoperatively denoted no significant difference in each group（P0.05）.The side effects and usage of meperidine within 24h postoperatively showed no significant difference between two groups（P0.05）.Conclusion：Flurbiprofen display significant analgesic effects and no influence on platelet aggregation for the patients undergoing LC.
