摘 要:目的∶为使异种骨移植应用于骨干节段性骨缺损治疗,作者将经去抗原处理的牛松质骨骨块与牛骨形态发生蛋白复合形成块形重组合异种骨(massivereconstitutedbonexenograft,MRBX),通过犬桡骨干中上段20mm骨-骨膜缺损修复模型,综合评价MRBX修复大型动物长骨节段性骨缺损的能力。方法∶6只成年犬左侧桡骨节段性骨-骨膜缺损处植MRBX,右侧不做修复,为空白对照。术后做X线摄片、同位素骨扫描及病理组织学检查。结果∶术后时间动态X线检查发现植MRBX侧骨缺损的修复在骨缺损两端呈现传导成骨的特征,新骨由骨端向植骨区长入;而植骨中部的修复则表现为诱导成骨的特征,植骨早期其内部出现多个软骨、骨岛样骨化中心,这些骨岛逐步连成一体,最后与两端的骨痂相接而完成骨缺损的修复。术后3个月,同位素骨扫描发现植MRBX侧有连续性的同位素显像增强,说明植骨中出现活跃的骨生长活动,而空白对照侧只在骨缺损的两个截骨端出现显像。术后6个月组织形态学观察,在新生骨形成、改建、成熟的同时MRBX被破骨细胞分解、吸收,最终被新骨所替代。结论∶实验结果提示,MRBX通过诱导成骨及传导成骨方式修复大型动物骨干节段性骨缺损,具有?bjective:The purpose of this study was to determine whether xenogenetic bone grafting could be used in the treatment of segmental bone defect. Methods: The 20 mm segmental radius bone/periosteum defect, a predictable non union model, was created in six adult dogs. Massive reconstituted bone xenograft (MRBX) was used for the treatment of the left segmental radius defect, while the opposite defect was kept untreated as a control. The formation and healing of bones were determined by serial radiographic, histological, and SPECT analyses. Results: The SPECT scanning showed the continuous image at the defects in MRBX group, while the image in control defects was located only at the two cut ends of the section. Five of the six defects in MRBX group had significant bone formation and union 6 months postoperatively, which was confirmed both by radiographic and histological evidence. On the contrary, no formation of cartilage or bone was observed in the central region in all the six control defects. Conclusion: The results of this study suggested that MRBX as a bone graft substitute of unlimited availability has great potentials for clinical use when segmental bone defect exists and local bone formation is needed.
